The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. The NCSB is seeking a passionate and dedicated Case Manager III to join the Same Day Access unit! In this role, the individual will provide care coordination to individuals enrolled in the NCSB mental health and substance use services or refer individuals to other community services should the NCSB not offer the appropriate services needed. The Care Coordinator will establish working relationships with community agencies and internal NCSB programs, reduce barriers for clients seeking mental health and substance use services, and guide individuals during the transition and enrollment into NCSB services. Departmental Hiring Salary Range: $46,589-$53,577
Essential Functions
Essential functions include but are not limited to:
+ Provide a safe and welcoming environment while providing care coordination services to consumers awaiting enrollment to NCSB mental health and/or substance use services or I-Care only clients.
+ Monitor consumers awaiting enrollment to NCSB mental health and/or substance use services for physical and mental stability, overall wellness, and consumer satisfaction through consistent face-to-face encounters and ancillary contact by phone and mail with consumers. Will provide increased frequency of care coordination services when clinically indicated.
+ Assess consumers for suicidal or homicidal ideations, intent, and/or plan throughout care coordination activities and while consumers are awaiting enrollment to appropriate NCSB mental health and/or substance use services.
+ Provide crisis counseling and intervention and/or utilize emergency services as needed upon assessment of consumers for threat or harm to self-and/or others.
+ Link and refer consumers to NCSB interim services, community resources, community providers, community agencies, and other support while they await enrollment to NCSB mental health and/or substance use services. Examples include DSS, Social Security Administration, external substance abuse treatment centers, specialized non-profit organizations, medical or primary care providers, vocational training, supportive housing, homeless services, etc.
+ Serve as a consumer advocate and establish a collaborative relationship with consumers and identified support system to maximize the consumer's ability to make informed decisions.
+ Coordinate and plan services with other providers within the NCSB and with other agencies such as probation officers, jail staff, psychiatric hospitals, counselors, vocational programs, physicians, etc.
+ Obtain and maintain clinical documentation and health records from other providers to ensure continuity of care.
+ Provide practical counseling and education to guide the individual in problem solving strategies and to develop a supportive therapeutic relationship. This may include encouraging compliance with treatment, substance abuse education, discussing relapse prevention strategies, stress management, etc.
+ Maintain timely, appropriate, and required documentation related to service delivery, including opening consumer records, in accordance with applicable ethical requirements and the requirements of the NCSB and other regulatory agencies (federal, state, and other).
+ Collect and report service statistics and consumer data as required or requested.
+ May assist in the development and implementation of special projects at the request of the Clinical Supervisor.
+ Participate in regular supervision with Clinical Supervisor and consult with other team members.
+ Attend agency staff meetings, case reviews, and training as required.

Education/Experience
Bachelor's degree in a human services field.
Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
Preferred Experience:
At least two years' experience mental health and/or substance use case management.
Additional Information & Requirements
+ Valid Driver's License may be required depending on assignment.
+ Certification in basic first aid and CPR, depending on area of assignment.

+ Retirement
If you are hired October 5, 2010, or after to a position with City of Norfolk Retirement System benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
+ The Tuition Assistance Program is established to encourage employees toward continued self-development and education. Permanent full-time and permanent part-time classified, unclassified permanent, full-time special projects, and constitutional employees who have completed six months continuous service will be eligible to apply. The applicant’s school of enrollment must be an accredited institution.
