Job Summary and Qualifications
Responsible for facilitating the Stroke Program with the goal of ensuring high quality, patient-centered care through the oversight and coordination of all systems related to acute stroke care delivery. The Stroke Program Manager will lead, monitor, and evaluate the Stroke Program to achieve optimal outcomes based on current evidence- based standard of care and act as a liaison between physicians, clinical staff, and senior leadership
Quality
+ Assists with continued growth and development of the Stroke Program by operationalizing evidence-based practices to achieve optimal and quality outcomes for the stroke patient
+ Coordinates care of acute stroke patients across the continuum of care by collaborating with the appropriate hospital departments, medical staff, and senior leadership
+ Ensures a culture of accountability and transparency in the provision of care to the acute stroke patient
+ Develops, reviews, and ensures compliance with policies and procedures in relation to the care of the stroke patient, consistent with evidence-based practice recommendations ensuring regulatory compliance of the Stroke Program
+ Assures continuous quality improvement through data collection, analysis, and process improvement using data from a variety of sources
Service
+ Provides leadership in attaining and sustaining exceptional patient experience through the provision of consistent, positive patient interactions for the stroke population
+ Collects and analyzes a variety of patient experience data to help identify opportunities to improve the experience of the stroke patient.
+ Ensures compliance with evidence-based practices and regulatory requirements that enhance the stroke patient experience
+ Supports a patient-first philosophy and engages in service recovery when necessary
+ Collaborates with the stroke team to develop standardized patient teaching materials
+ Collaborates with community partners and outreach to ensure community awareness of stroke signs and symptoms through education and health screening opportunities.
People
+ Leads in a way that inspires a commitment to the mission and values of the organization and of the acute stroke program
+ Develops and provides education to staff and physicians on evidence-based practice guidelines including assessment, treatments, and regulatory standards compliance for the care of the stroke patient
+ Collaborates with department and administrative leadership to evaluate the educational needs of the staff and medical staff and implements plans to meet those educational needs related to the stroke program.
Growth
+ Supervises data specialist to ensure accurate data collection and application of data statistics to grow the stroke program at all campuses of Henrico doctors’
+ Demonstrates personal growth and leadership by participating in and facilitating departmental and hospital committees and professional organizations involved in the direction of improvements in the care of the stroke patient
+ Communicates effectively, both written and verbally with all members of the healthcare team and other key stakeholders.
Finance
+ Evaluates program needs including equipment and supplies needed to provide optimal care to the stroke patient.
+ Identifies cost reduction and opportunities as appropriate
What qualifications you will need:
+ Required – Graduate of an accredited school of nursing. Preferred Master’s Degree in Nursing
+ RN with at least 5 years experience in an acute care setting with knowledge of stroke treatment and management.
+ Required Current Virginia State licensure by the State Board of Nursing as a Registered Professional Nurse or compact license.
+ Preferred Specialty Certification.
