    Phoenix Home Care & Hospice is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide compassionate, skilled 1:1 care in the homes of our clients in Hurley, MO.     Job Responsibilities:

    + Follow the individualized plan of care developed with the physician

    + Monitor, document, and report the patient’s condition using electronic charting

    + Educate and provide support to the client and their family

    + Attend team meetings and training sessions as required

    Qualifications:

    + Active State of Missouri or Compact Nursing License (LPN or RN)

    + Strong communication and critical thinking skills

    + Compassionate, professional, and positive attitude

    + Ability to work independently and manage time effectively
