Lallie Kemp Regional Medical Center is seeking a Licensed Practical Nurse to join our dedicated healthcare team.
Lallie Kemp Regional Medical Center is a 24-bed acute care critical access hospital serving the Florida Parishes region of Southeast Louisiana. We offer a wide range of services including general surgery and a 24-hour emergency department. Minimum Qualifications
Possession of a current Louisiana Practical Nurse license, a temporary permit, or a multi-state license issued by a Nursing Licensure Compact (NLC/eNLC) state.

Job Duties and Other Information
Job Duties:
40% Implementation
Ability to handle the required duties and tasks to provide coverage to Oncology, Cardiology, Surgery, GYN and Infectious Disease clinic and other specialty clinics on a moment's notice as well as LPN nursing care duties located in other Outpatient areas.
Responsible as the Lead LPN in the Clinics by providing nursing care setting up clinic rooms and assisting Physician or Nurse Practitioner.
Provides Blood Pressure skills training to Medical Specialist.
Implement plan of care as established by the physician and interdisciplinary team members. Establish priorities based on acuity of health care needs when caring for assigned patients.
Demonstrate flexibility as patient care needs change.
Accurately transcribe orders, perform procedures, administer medications, and observe changes in condition during clinic visit. Reassesses patient per nursing policy/procedure following treatment delivered during the clinic visit, communicate with intra-hospital departments and community resources as necessary to provide ordered services.
Document delivery of nursing services, patient response and completes required paperwork for billing purposes per outpatient clinic, nursing, compliance and facility policy/procedure.
Implement and document patient education and teaching with patient, family, relatives, friends, or others the patient has identified who are involved with their health care.
Provides verbal and written educational materials in the patient's primary language.
Utilize facility approved interpreter for hearing impaired patients and facility services such as Language Line Services or foreign language interpreter services to effectively communicate with patients when needed.
Maintain a safe environment by always observing infection control and risk management guidelines, recognizing, correcting and/or reporting breaches in infection control and safety hazards.
Complete ABN's and other requirements for Medicare patients.
Complete referrals related to patient healthcare plans including but not limited to Home Health, Hospice, Community Care, other LSU/HCSD facilities referrals and community facilities. Maintain original copy of referral in the patient's medical record.
Establish effective interpersonal relationships with patient, family, relatives, friends, or others the patient has identified who are involved with their healthcare and co-workers to create a professional and caring environment and to accomplish the goals of the established individualized plans of care.
Provide patient counseling with patients, family, relatives, friends, or others the patient has identified who are involved with their health care, within the scope of the LPN license.
20 % Assessment:
Participate in assigned patient health evaluation at the time of each clinic visit by assessing as a minimum the vital signs, Height, Weight, BMI, pain, and obtaining from the patient and/or medical record and/or from the family, relatives, friends, or others the patient has identified who are involved with their health care, the dates of the last PSA, Mammo, PAP, as appropriate for sex and age of patient, smoking status, possible abuse, medical and surgical history, pertinent family history, immunization history and current medication list.
Assess immediate health needs and abnormal condition of assigned patients. Participate in the evaluation of the need for assigned patients/significant others health education. Use clinical experience and knowledge to collect data and anticipate potential patient care problems.
Reassess patient when indicated by change in acuity and urgency of patient's condition during the clinic visit. Document assessment activities according to facility charting and documentation standards.
Advise team leader, RN supervisor or attending physician of abnormal assessment findings that may require immediate intervention and/or as required by policy/procedure.
20% Planning:
Utilize assessment data to determine the overall needs of the patient.
Participate with the physician and interdisciplinary team members as indicated to develop individualized patient goal(s).
Collaborate with the physician, team leader and interdisciplinary team members as indicated to plan health education strategies with patient, family, relatives, friends, or others the patient has identified who are involved with their health care.
10% Evaluation
Assess the immediate effectiveness of medication administered or treatment provided during the clinic visit. During subsequent visits evaluate the patient compliance with plan of care and the long term effectiveness of the medication regime, patient education, referrals to other facilities, etc.
Participation in the re-evaluation of patient's needs at each clinic visit.
10% Other:
Input required data into the electronic health record (EPIC) maintenance and other required programs per policy.
Adhere to all HIPAA/compliance regulations including reporting non-compliance issues involving self and/or others, to TJC guidelines, to the facility, nursing and outpatient clinic policies and procedures, as well as to HCET guidelines.
Actively participates in HCET workgroups and unit Quality Performance Improvement. Maintain a neat and organized work area and is compliant with the clinic cleaning assignment. Perform in a calm and professional manner during crisis or emergency situations.
Documents on an occurrence form any out of the ordinary occurrence and report to team leader, RN Supervisor A or RN Outpatient Manager.
Accept preceptor assignments as delegated.
Assist in the proper maintenance of machinery and reports building and equipment problems/malfunctions to RN Supervisor A, Team Leader or RN Outpatient Manager immediately.
Attend unit meetings as scheduled unless previously excused.
Complete assigned unit and facility in-services.
Assist in other areas of the outpatient clinic when assigned work is completed. Adhere to the clinic schedule, work assignment and closing schedule.
Competently work in all Outpatient Clinics following initial orientation.
When assigned by the supervisor, adjust staff assignments to meet patient care needs.
Assist with providing phone reminders of upcoming appointments to patients for the practitioners to which she/he is assigned.
Education and Training
Attend Educational In-Services, Mandatory Training and Annual Review as required by hospital policy and procedure or deemed necessary by supervisor. Any and all other duties as assigned include ensuring departmental coverage.
This incumbent shall possess and maintain a current Louisiana LPN License as well as a valid Basic Life Support certification form the American Heart Association.
Position Specific Details:
Work Location: This LPN position is located in the Outpatient Clinic.
Work Schedule: Shifts will be determined by the department manager.
Career Progression: As part of a Career Progression Group, vacancies may be filled from this recruitment as a Practical Nurse Licensed 1 or2 depending on the level of experience of the selected applicant(s).
Compensation: The following is a Special Entrance Rate (SER) for the LPN 1 or 2 Vacancy within Outpatient Clinic:
- LPN 1 – Starting Salary is $19.48/per hour worked + Shift Diffs.
- LPN 2 – Starting Salary is $20.84/per hour worked + Shift Diffs.
