    Salary: $950 / Week

     

    Home Health Physical Therapist Assistant – Centralia, WA98531 (Full-Time)

     

    Contract Type: Flexible (26 weeks to 1 year, orTemp-to-Perm)

     

    Schedule: Full-Time, 40 hours/week

    Travel Pay Package:

    + Hourly Rate: $18/hour (40 hours/week)

    + Meals Stipend (Non-Taxed): $476/week

    + Housing Stipend (Non-Taxed): $770/week

    + Total Gross Weekly: $2,006

     

    Local Pay: $40–$42/hr

    Position Summary:

    We are seeking a compassionate and skilled HomeHealth Physical Therapist Assistant (PTA) to join our team inCentralia, WA. This role is perfect for a motivated clinician who enjoysworking independently and making a difference in patients’ lives by helpingthem regain mobility and independence in the comfort of their homes.

    Key Responsibilities:

    + Provide physical therapy services under the direction of a licensed Physical Therapist

    + Assist in implementing individualized care plans

    + Collaborate with interdisciplinary teams to ensure quality care

    + Accurately document visits and patient progress using Homecare Homebase (preferred)

    + Meet a productivity expectation of 26 points per week

    Qualifications:

    + Licensed Physical Therapist Assistant in the state of Washington (required)

    + Minimum 1 year of experience as a PTA (required)

    + Experience with OASIS documentation and Homecare Homebase (preferred)

    + Valid Driver’s License and reliable transportation

    + Must meet onboarding requirements (see below)

    Onboarding Requirements:

    + Background Check

    + Proof of Auto Insurance

    + WA PTA License Verification

    + BLS Certification

    + Driver’s License

    + TB Test

    + 9-Panel Urine Drug Screen

    + Hepatitis B Vaccination

    + Physical Exam
