CHMC – MHU- Treatment Coordinator – Per Diem- Day Shift - Requisition 8765

Pay Range: $26.40-$34.31*

A Mental Health Treatment Coordinator plans and evaluates patients’ care, prepares treatment plans, updates and collaborates with a multidisciplinary team, applies psychotherapeutic techniques in a group setting, conducts family and patient psycho education, facilitates family participation, contacts outpatient providers to facilitate the continuity of care in the inpatient setting as well outpatient follow-up upon discharge.

Qualifications:

+ Knowledge, understanding, and ability to work with mentally ill patients required.

+ Basic computer knowledge, maintaining confidentiality of all patient information and maintaining quality assurance and compliance with all regulatory agencies required.

+ Ability to implement and support procedures and program pertaining to patient safety and security.

+ Exhibit proficient communication skills and ability to deal efficiently and effectively with a variety of personalities.

+ Ability to sustain attention to detail and accuracy as it pertains to the monitoring and documentation of information. Maintaining a safe and clean environment for patients.

+ Bachelors degree in Psychology required. Masters degree in Social Work preferred.

+ BLS certification and PMCS training required.

+ Valid New York State Master Social Worker license, if position is held by a LMSW.

*The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
