    Intake/Home Care Coordinator

     

    Full Time - 1.0 FTE

     

    Day Shift - Monday - Friday 8:00 AM - 4:30 PM (in office)

    Remote On-Call:

    + Rotation every 5-6 weeks

    + Monday - Friday 4:30 PM - 10:00 PM

    + Saturday - Sunday 8:00 AM - 10:00 PM

    Responsibilities

    + Coordinate In-Home Care Services – Schedule initial home care appointments, arrange equipment delivery, and obtain insurance authorizations. Utilize the home care coordination team to ensure seamless service for patients and referrals.

    + Manage Patient Intake & Communication – Process incoming referrals, gather demographic and financial information, verify insurance benefits, and direct inquiries to the appropriate service lines. Maintain clear and professional communication with patients, referral sources, and third-party payers.

    + Ensure Compliance & Customer Satisfaction – Maintain knowledge of regulations, follow up with patients, and provide excellent customer service. Assist with team coordination as needed, participate in on-call rotations, and uphold UnityPoint at Home’s reputation as a high-quality care provider.

    Qualifications

    + One year medical experience/previous health care or home care related Intake/Coding + customer service experience

    + High-school diploma
