POPULATION HEALTH PHARMACY TECH
Position Summary:
The Saint Alphonsus Medical Group (SAMG) Population Health Pharmacy Program supports Population Health initiatives to increase the quality of patient care while maintaining STAR measure outcomes through disease and medication management programs with the payor.
The Pharmacy Technician is an integral member of the SAMG Population Health team and will work closely with the Pharmacist to execute strategies for the Population Health Pharmacy Value-Based Care incentive programs. The Pharmacist and Technician team will improve patients’ medication adherence and work together to implement changes to improve quality of patient care.
The Population Health Pharmacy Technician will assist the Pharmacist in monitoring and educating patients on medication adherence and disease management to ensure their health goals are met. The primary goal of this role is to enhance performance on Population Health measures related to medication adherence in specific areas of diabetes, hypertension, and hyperlipidemia.
Performance will be tracked monthly, quarterly, and yearly by Medicare STAR quality ratings. The Pharmacy Technician is also responsible for gathering information from medical records, excel spreadsheets, and other technology-based systems to support quality measure performance.
Requirements:
+ Current and Valid Idaho Board of Pharmacy Technician licensure in good standing
+ Excellent verbal and listening skills on the phone. Capable of communicating prescription authorizations to pharmacists/medical staffs/pharmacies.
+ Ability to utilize Excel and other Microsoft programs.
+ Able to use pharmacy information and gather patient data on EPIC EMR (or willing to train / learn efficiently).
+ Ability to establish and maintain cooperative and collaborative working relationships with pharmacists, providers, nurses, and other clinical staffs.
+ Demonstrate writing skills to compose logical, detailed and grammatically correct correspondence via electronically
+ Knowledge of efficient interviewing practices and techniques used in order to gather information from patients, pharmacy staffs and medical staffs
+ Ability to remain calm and be resourceful in complex situations over the phone.
+ Ability to maintain a professional demeanor at all times.
