    Description:

    The Social Care Division Coordinator provides essential support and coordination of referral intakes for the CHW program, the Farm, and the DPP program, ensuring efficient operations and effective client services across Trinity Health Michigan (Statewide). This role involves scheduling patient appointments in EPIC and ensuring close communication and coordination with the call center and team leaders to ensure patients are served in a timely manner. The role will also administratively support the social care division by organizing and pulling data and metrics, performing chart audits, and preparing client reports. Additionally, the coordinator will utilize Peoplesoft to ensure invoices are paid in a timely manner.

    POSITION PURPOSE

    The Social Care Division Coordinator provides essential support and coordination of referral intakes for the CHW program, the Farm, and the DPP program, ensuring efficient operations and effective client services across Trinity Health Michigan (Statewide). This role involves scheduling patient appointments in EPIC and ensuring close communication and coordination with the call center and team leaders to ensure patients are served in a timely manner. The role will also administratively support the social care division by organizing and pulling data and metrics, performing chart audits, and preparing client reports. Additionally, the coordinator will utilize Peoplesoft to ensure invoices are paid in a timely manner.

    POSITION QUALIFICATIONS

    **High School graduation required** .

     

    Some college, Associates Degree or Bachelor's Degree form an accredited college or university is preferred in public administration, health education, communications, marketing, sociology, social work, planning or closely related field is desirable.

     

    + At least 2 years' experience working in nonprofit or community-based initiatives, education or government with officials, staff, volunteers and clients from a broad range of community sectors.

    + Experience with program or project coordination. Experience in a community-based or healthcare setting preferred.

    + Excellent organizational skills, effective written and verbal communication skills. Able to work effectively and efficiently under tight deadlines. Proficiency in Microsoft Office programs.

    + Demonstrates leadership and teaching abilities, including group facilitation skills. Ability to make quality, independent decisions. Able to handle confidential information.

    + Must have a good sense of humor and an ability to get along with a wide variety of people; must be comfortable taking initiative, but also acting in a supporting role; must be comfortable with direct advocacy and community engagement.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

    1. Maintains good rapport and cooperative relationships. Approach conflicts in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.

    2. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Trinity Health. Discusses patient and hospital information only among appropriate personnel in appropriately private places.

    3. Behaves in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values and policies of Trinity Health.

    4. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

    5. Answer and manage client phone calls, addressing social needs, obtaining necessary documents, and resolving hospital-specific inquiries.

    6. Add clients to the CHW referral work queue, ensuring they have existing charts and creating profiles for new patients.

    7. Verify client enrollment status, adding new clients to the program, referring them to the CHW program, and assigning patient to appropriate template in EPIC.

    8. Collect, analyze, and report data related to CHW program activities and outcomes, maintaining client and program information databases.

    9. Assist with training and onboarding of new CHWs, providing ongoing support and resources to the team.

    10. Ensure compliance with organizational policies and procedures, conducting quality assurance checks to maintain high standards of service delivery.

    11. Ability to work closely with Team Leaders to identify potential scheduling needs or changes.

    12. Ability to intake referrals from hospital and outpatient providers across the health system in the State of Michigan. Ability to work closely with the call center to intake and schedule patients appropriately.

    13. Ability to create scheduling templates that have appropriate time allotted to accommodate patient needs.

    14. Ability to support the DPP program, the Farm, and the CHW program by scheduling appointments and processing intakes from the call center within 24 hours.

    15. Ability to run monthly data reports, including but not limited to compliance reports and audits that support the social care division.

    16. Perform additional tasks and responsibilities as assigned by the Director or managers, contributing to the overall success of the CHW program.
