The Arc Erie County is looking for a Coordinator of Central Intake!
What you’ll be doing: As our Coordinator of Central Intake, you will be responsible for the oversight of all aspects of centralized, single point of entry admissions for The Arc Erie County New York’s supports and services.
You will be responsible for…
+ For referrals made to the Education Program, meets with family/caregiver(s) in completing paperwork necessary for the child’s enrollment; Monitors and coordinates submission of paperwork to respective municipality/ school district/departments
+ Creating & maintaining the implementation of all applicable program expectations, policies & procedures about Intake, Admissions, Discharges, etc., updates these as necessary
+ Working in conjunction with public relations to create, maintain, and update as necessary the information with regards to supports and services offered
+ Monitoring program utilization with regards to admissions/discharges
+ Participates and follows up on the assignment of all referrals and intakes. Evaluates eligibility and secures appropriate documentation.
What we need from you:
Master’s Degree in a human service field degree plus (2) yrs direct service experience w/ developmentally disabled individuals OR
Bachelor’s Degree in a human service field degree plus (3) yrs direct service experience w/ developmentally disabled individuals OR
High School diploma or equivalent plus (5) yrs direct service experience w/ developmentally disabled individuals

Position Summary:
The Coordinator is responsible for the oversight of all aspects of the centralized, single point of entry admissions for The Arc Erie County’s supports and services. This includes, but not limited to: eligibility, assessment, enrollment and discharge, linkage and referral, monitoring of individualized program planning, compliance with Federal, State, County, and Agency regulations and program fiscal affairs, staff hiring, supervision, training, performance evaluations, incident management, and development and ongoing revision of programs policies and procedures.
Reporting Relationship:
Under the direct supervision of the Associate Director of Outreach. Directly supervises Intake Specialists as assigned.
Job Duties and Responsibilities:
+ General Duties:
+ + For referrals made to the Education Program, meets with family/caregiver(s) in completing paperwork necessary for the child’s enrollment; Monitors and coordinates submission of paperwork to respective municipality/ school district/departments; Coordinates preschool evaluations with families and multidisciplinary team as required; As part of the evaluation team, completes social service history; Attends initial Committee on Preschool Special Education meetings as necessary as a representative of the evaluation team; Routes demographic information regarding incoming students to pertinent staff.
+ Assists in the creation and maintenance of the implementation of all applicable program expectations, policies, and procedures with regards to Intake, Admissions, Discharges, etc., and updates these as necessary.
+ Monitors program utilization with regards to admissions/discharges
+ Participates and follows up on the assignment of all referrals and intakes. Evaluates eligibility and secures appropriate documentation.
+ Collects and analyzes data with regards to programmatic admissions, discharges, change of vendor in/out, program census information, referrals, denials, waitlists and access to services.
+ Identify community needs and refer to program management for further development.
+ Identify and cultivate stakeholder relationships to support engagement and agency growth
+ Plan, organize, and execute outreach events
+ Ensure events are inclusive, welcoming, and reflect the agency's mission and values.
+ Develop and maintain relationships with schools, and other relevant organizations.
+ Maintain a professional relationship with area school districts to include Committee on Special Education Chairpersons, School Psychologists, and Special Education Teachers
+ Serve as a liaison between the agency and the broader community
+ Provides a comprehensive report to senior management and the Board of Directors as assigned with regards to Intake/Admissions.
+ Provides management with programmatic recommendations based on data and population needs
+ Represents agency on inter-agency committees, as assigned.
+ Works cooperatively in a team approach to meet agency, program and individuals needs as appropriate.
+ Develops and maintains professional relationships with collaterals and other administrative personnel of similar community agencies.
+ Organizes and/or participates in staff meetings, trainings, and professional workshops, as assigned.
+ Participate in the interviewing, hiring, and training and evaluation of assigned staff. Assesses, coordinates, and monitors staff training on an on-going basis
+ Other duties as assigned.
+ Supports/Services:
+ Knowledge of all The Arc Erie County New York’s supports and services, including but not limited to OMH, OPWDD, and the Department of Education
+ Maintains awareness of generic and specialized services and supports available to persons with developmental disabilities. Maintains knowledge of service continuum and how to access it. Promotes natural supports in the community.
+ Provides information and consultation concerning developmental disabilities to community agencies and individuals as requested.
Qualifications and Education Requirements:
Bachelor’s Degree, including but not limited to: Sociology, Special Education, Rehabilitation Counseling and Psychology PLUS three years of direct service experience with the developmentally disabled with at least one year in a supervisory capacity.
OR
Associate’s Degree, including but not limited to: Sociology, Special Education, Rehabilitation Counseling and Psychology PLUS five years of direct service experience with the developmentally disabled with at least two years in a supervisory capacity.

The ability to operate medical equipment such as nebulizers, suction devices and the like; ability to operate office equipment, such as computer, copier, fax machine, and laminator
