Job Title: Social Worker - Pediatric/Adult Metabolic Service
_Social Work Services at the Mount Sinai Hospital_
The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.
+ The social worker in this position will provide social work services to patients and their families from birth into adulthood that are diagnosed with an inborn error of metabolism.
+ Inborn errors of Metabolism require life-long management and treatment with specialized diets and medications.
+ Some disorders are prone to catastrophic crises while others are more chronic in nature.
Qualifications
+ NYS LMSW or LCSW.
+ Excellent communication, collaborative, and team leadership skills.
+ Excellent organizational, data collection and documentation skills.
+ Demonstrated experience providing psychosocial support and counseling to patients/families with chronic & complex illnesses including interventions related to death and dying and bereavement counseling in health care settings.
+ Willingness to learn complex medical information related to rare diseases.
+ Experience with complex care coordination activities to facilitate multidisciplinary care of high-risk metabolic population.
+ Working knowledge of childhood development as affecting clinical interventions.
+ Strong interest in programmatic development including expansion in areas of social media.
+ Bilingual (English/Spanish) _preferred._

Responsibilities
+ The social worker will provide short and long-term individual and family care through psychotherapy, care management, crisis intervention, and health education as an active member on the interdisciplinary medical team.
+ The social worker will follow these patients both in the outpatient clinic setting as well as the pediatric inpatient setting where admitted (including: NICU, PICU, P4 & P5).
+ This social worker will work with families whose child is picked up on New York State Newborn Screening at birth and follow them into adulthood, providing emotional support and arranging care at each major childhood stage.
+ There can be end-of-life and bereavement care associated with this role and the social worker should be prepared to support the patient/family through the process.
+ Strong advocacy in keeping with a patient’s evolving medical and developmental needs strongly underpins all aspects of this work.
+ When adult patients are hospitalized, the social worker in this position will liaise with the unit social worker.
+ This includes, but is not limited to, helping with unique metabolic specific resource needs, medications and formulas.
+ Approximately 30% of clinic patients are adults; 70% pediatric.
+ The social worker will be expected to attend clinic in person as requested by the Metabolic Team.

Compensation Statement
Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $85280 - $97760.36 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
