    Join UPMC Hamot Health Foundation as our Director of Community Health Initiatives and be at the forefront of transforming community well-being!

     

    As a vital leader within our Foundation, you will champion initiatives that tackle the social determinants of health by fostering strong, lasting relationships across the regions we serve. Partnering closely with the Chief Development Officer, you will drive impactful efforts and lead community grant programs designed to deliver measurable results in healthcare access, housing stability, food security, career development, and neighborhood outreach.

    In this role, there is the opportunity to -

    + Participate actively in marketing UPMC Hamot Health Foundation (HHF) services and programs through public speaking, press events, and personal outreach.

    + Apply management principles to develop personnel and optimize program performance.

    + Assure sound fiscal operations by preparing timely, accurate, and comprehensive annual budgets.

    + Utilize reporting tools to monitor and enhance the quality, safety, efficiency, and effectiveness of mission-related initiatives.

    + Demonstrate a strong commitment to patient care and community engagement through personal behavior and leadership.

    + Develop and implement strategic plans aligned with HHF’s mission, vision, and values.

    + Manage community grants by developing expertise in grant administration to strengthen patient and community partnerships.

    + Build and maintain relationships with community and government entities to foster collaboration across all service areas.

    + Ensure initiatives meet population needs through ongoing assessment, monitoring, and adjustment.

    + Implement financial controls and reporting processes to maintain fiscal responsibility and meet organizational objectives.

    + Promote positive working relationships within the department and with external agencies to enhance cooperation.

    + Provide effective communication channels for all internal and external customers of UPMC Hamot Health Foundation.

    + Model leadership behaviors that foster employee engagement, positive attitudes, and active involvement in community initiatives.

    + Support the Chief Development Officer in advancing community benefit, diversity, and inclusion initiatives across HHF and affiliated entities.

    + Research, design, and implement community health improvement objectives in collaboration with HHF staff and partners.

    Required

    + Bachelor's degree

    + 5 years of effective management experience in a related health care setting

    + Through knowledge of health care delivery systems, practice management, health care finance, community benefits and facilities management.

    + Demonstrate knowledge of management techniques, the nursing process, interpersonal relationship skills and group dynamics

    + Express self clearly in writing, speaking and manner

    Preferred

    + Advanced degree strongly preferred (MHS, MBA, MSN, MPH, or equivalent)
