Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens) and Gurwin Certified and Licensed Home Care Services Agencies.

Our Adult Day Care Program is seeking a Program Aide to work Monday through Friday from 8:30am to 4:30pm with rotating weekends who exemplifies the passion and commitment that is required to deliver the ultimate care for our Adult Day Care participants.

    EDUCATION AND EXPERIENCE REQUIREMENTS:

    + Certified in New York State Nurse Aide Registry (not expired or lapsed) preferred.

    + Candidate must have no findings or convictions of resident abuse, mistreatment, neglect, or misappropriation of resident property listed on the Registry against them.

    + One year long term care experience and CPR certification preferred.

    DUTIES AND RESPONSIBILITIES:

    + Provides services based on TR Service Model.

    + Keeps accurate statistical reports of activities offered and level of participation for person served.

    + Performs other related miscellaneous duties as requested (such as assist in TR programs, chaperone on community trips, transport registrants to from activities and run errands, etc.

    + Actively shares knowledge, foster and promotes dissemination of information as it relates to organizational needs.

    + Conveys information to treatment team in a timely manner regarding person served.

    + Keeps bulletin boards up to date.

    + Assists PT in group and individual exercises, transport registrants to and from PT and other duties as needed.

    + Maintains current lists of registrants for special programs.

    + Attends staff conferences as requested.

    + Attends in-service education programs as requested.

    + Demonstrates ability to drive facility recreational van.

    + Capable of lifting and carrying equipment and materials.

    + Capable of taking attendance and assisting during dismissal.

    + Capable of transporting residents to and from recreation area.

    + Perfoms other duties as needed.
