    POSITION SUMMARY:

    Health Information Management HIM III Specialist reports to the Manager, Health Information Management and supports all basic functions of HIM. This position is accountable for reviewing, analyzing and assembling medical record documentation (automated and hard copy) and ensuring accuracy of record types, demographic data, including following-up on deficiencies in documentation according to departmental policies and procedures. The position is also responsible for work assignment and production for Incomplete Record Analysis and Provider Communications both written and electronic. Provides direct production assistance as needed to address staffing shortages and as crossed trained. This position scans single and batch documents/records to the EMR. Manually index non-barcoded documents, reviews and identifies image quality and indexing issues. Performs EMR quality validations, routine maintenance and adjust settings on the scanning equipment. Performs computer functions in a Microsoft Windows environment. Maintains record control using automated systems. Researches and provides records for clinical areas daily. Monitor daily provider document deficiencies and communicates to providers both in written and electronic form.

    EDUCATION/CERTIFICATION:

    This position requires a High School Diploma or GED.

    EXPERIENCE:

    • Two years of hospital and/or medical office experience with related field preferred.

    COMPETENCIES:

    • Basic computer skills are necessary for daily communications and use of Microsoft windows based programs such as Outlook, Word, Excel and internal Intranet and Meditech programs.

    • Must be able to read, speak and write proficiently in English in order to effectively communicate on a daily basis with hospital staff, patients and visitors.

    ESSENTIAL DUTIES and RESPONSIBILITIES:

    1. Notes any and all provider deficiencies electronic and paper. Completes electronic deficiency notice with 95-100% accuracy.

    2. Reviews in-patient and ambulatory records. Completes deficiency analysis timely and within daily production standards.

    3. Ensures the appropriate reports and signatures have been designated to be completed by provider with 95-100% accuracy. Refers cases in question to supervisor.

    4. Reviews all records subsequent to provider completion to verify appropriate reports and signatures, previously noted have been completed at a level of 95-100% accuracy. Refers cases in question to supervisor.

    5. Monitors daily OR schedule. Notes deficiencies for non-compliant Operative dictation and notifies Physician office. Documents misdirected faxes.

    6. Monitors daily electronic signature reports. Updates electronic deficiency assignment.

    7. Completes weekly incomplete record notices, warnings and suspension to providers. Maintains notice file. Reports weekly notices to HIM Records Manager.

    8. Reports weekly incomplete charts to HIM Records Manager.

    9. Responsible for the process of the electronic patient record documentation completion by identifying all deficiencies, reviewing, analyzing hybrid medical record (electronic and paper based), identifying and following through on documentation deficiencies

    10. Assist Physicians with EMR completion. Strong knowledge of medical record format and content for inpatient and outpatient encounters.

    11. Demonstrates an understanding of Release of Information rules and regulations. Adheres to established departmental policies and procedures.

    12. Provides all basic functions of the HIM Spec I & HIM Spec II as needed for coverage.

    13. Recommends and supports changes in work process to achieve continued quality improvement

    14. Completes entry of daily productivity/task log. Maintains logs for quarterly review.
