Summary The HUD-VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and Federal agencies. In 1992, the Departments of Housing and Urban Development (HUD) and Veterans Affairs (VA) established the HUD-VA Supportive Housing (HUD-VASH) Program. The primary goal of HUD-VASH is to move Veterans and their families out of chronic homelessness. Responsibilities This is an open continuous announcement and will remain open until September 30, 2025. The initial cut-off date for referral of eligible applications will be 8 days from opening of announcement, with subsequent cut-off dates. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled. Once selections are made, the announcement will be closed. The HUD-VASH Social Worker will be responsible for coordination of case management services for Veterans receiving housing provided through HUD Section 8 housing vouchers. They must be clinically knowledgeable to provide and complete assessments with understanding of complex and diverse populations. A primary component of the program is VA case management, which is designed to improve Veterans' health and mental health, and to enhance their ability to remain stable, housed, and community integrated. Duties and responsibilities include, but are not limited to: Providing case management for a caseload of Veterans in the HUD-VASH program. Reviewing agreements between Section 8, landlords, Public Housing Authorities (PHAs), and the VA in establishing appropriate referral sites for HUD-VASH placements. Performing home visits in the community to ensure that Veterans reside in safe environments in compliance with local housing codes. Conducting community-based visits, made with safety in mind, and following the Home Visit Safety policy. Verifying the Veteran's status, eligibility for VA medical care, and eligibility of program participation. Ensuring that homeless Veterans are considered as primary participants for program acceptance. Independently completing all required documents for the HUD-VASH Program including those which are included in the data collection for NEPEC and within HOMES. Participating in ongoing performance improvement for the HUD-VASH Program including activities related to program specific performance measures. Providing HUD-VASH case management to Veterans in case management stages of Maintenance and Preparation for Discharge. Continuously assessing Veterans' needs and review case management phase with team and supervisor. Identifying housing options in the community while coordinating with landlords and property managers to negotiate the needs of the Veterans in the program. Documenting independently in the medical record, including psychosocial assessments, diagnostics, treatment planning, progress notes, follow up and referral and documents in the HOMES system. Total Rewards of a Allied Health Professional Work Schedule: The primary tour of duty may be Monday - Friday, 7:30am - 4:00pm, 8:00am-4:30pm, or 8:30am-5:00 pm. English Language Proficiency: You must be proficient in spoken and written English as required by 38 U.S.C. 7403(f). Education: You must possess a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. For assignments at the GS-9 level, VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent, master's level within three years of their appointment as a social worker. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. GS-9 Social Worker Experience, Education, and Licensure. None beyond the basic requirements. GS-11 Social Worker Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies. Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology GS-11 Knowledge, Skills, and Abilities: Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, educational, and other backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Preferred Experience: Social work experience with homeless or other vulnerable populations, like severe mental illness, substance use disorders, geriatrics, and community-based work. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-9 to GS-11. Physical Requirements: The position requires performance of work which involves light physical effort, such as that involved in lifting light files, bending, standing, reaching, operating office equipment such as telephone, copier, fax machine, scanner, etc. The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. In addition, must meet occupational health requirements for driving physical to include approval to transport Veterans in a government vehicle.
