The Clinical Coordinator is responsible for providing individualized habilitation services to adults with developmental disabilities, emphasizing rehabilitation-focused planning and implementation. This position ensures that each participant receives person-centered, outcome-driven support that promotes independence, skill development, and community integration. The Clinical Coordinator supervises program staff, coordinates individualized plans, and ensures the effective delivery of habilitative services in alignment with organizational policies and regulatory standards.
Key responsibilities include:
+ Conducts comprehensive assessments to evaluate each participant’s strengths, abilities, and support needs to guide the development of individualized habilitation goals.
+ Oversees the creation and biannual review of Day Habilitation Plans (DHPs), ensuring their effective implementation by program staff to support rehabilitation outcomes and participant progress.
+ Facilitates participant engagement in habilitation activities and community integration opportunities that promote functional skills development and independence.
+ Develops individualized staff action plans for each program participant and ensures documentation of monthly progress towards outcome attainment.
+ Collaborates with interdisciplinary teams to develop and write functional and self-directed outcomes within the Day Habilitation Plan, in coordination with the Individualized Service Plan (ISP) and Comprehensive Functional Assessment (CFA).
+ Provides training, guidance, and supervision to program staff to ensure accurate and consistent implementation of individualized habilitation plans and intervention strategies.
+ Identifies and coordinates community-based activities and skill-building opportunities aligned with each participant’s habilitation goals, ensuring appropriate supports, staffing, and equipment are available.
+ Monitors and evaluates the quality of habilitation services to ensure compliance with organizational policies, regulatory requirements, and person-centered planning practices.
+ Assesses the appropriateness of each participant’s placement within the day program and provides recommendations to program leadership to optimize rehabilitation outcomes.
+ Oversees day-to-day program operations to ensure safe, effective delivery of habilitation services and a supportive environment for all participants.
+ Maintains a safe and inclusive environment conducive to personal growth and skill development.
+ Communicates regularly with residential staff and family members regarding participant progress and coordinates ongoing care and services.
+ Integrates person-centered planning approaches and rehabilitation principles into all aspects of program operations, ensuring participant goals and activities support independence, self-direction, and quality of life.
+ This position requires a Bachelor’s degree in Counseling, Psychology, Sociology or a closely related field.
