Phoenix Home Care & Hospice is seeking Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide compassionate, skilled 1:1 care in the homes of our clients in Centralia, MO. Job Responsibilities:

+ Follow the individualized care plan developed with the physician

+ Monitor, document, and report patient conditions using electronic charting

+ Educate and support the client and their family

+ Participate in team meetings and required training

Qualifications:

+ Active Missouri or Compact State Nursing License (LPN or RN)

+ Compassionate and professional approach to care

+ Strong communication and critical thinking skills

+ Ability to work independently and manage time effectively
