    Your Role:

    + Provide skilled, compassionate nursing care in the home

    + Follow a personalized care plan in coordination with a physician

    + Monitor, document, and communicate patient status using electronic charting

    + Offer education and emotional support to patients and their families

    + Collaborate with your care team and participate in training

    Qualifications:

    + Active LPN or RN license in Missouri or Compact State

    + Availability for overnight and/or weekend shifts

    + Strong communication and clinical skills
