    About Grand Junction Regional Center:

    Grand Junction Regional Center (GJRC) is a collection of group homes that serves adult individuals with intellectual and developmental disabilities (IDD) who need additional support to stabilize and return to community residential settings. GJRC's focus is on improving outcomes for individuals with IDD as well as promoting community integration. GJRC has a total of 10 homes nestled throughout established neighborhoods of Grand Valley. These homes feature warm, comfortable living spaces and private, fenced backyards with recreation equipment.

    Description of Job

    The Quality Assurance Investigatoris responsible for ensuring compliance with agency, state, and federal regulations to protect the health, safety, and rights of individuals with intellectual and developmental disabilities. This role focuses on the thorough investigation and prevention of Mistreatment, Abuse, Neglect, and Exploitation (MANE) while supporting high-quality services across residential and day program settings.

    As a Quality Assurance Investigator, you can expect to:

    + Conduct independent investigations into allegations of Mistreatment, Abuse, Neglect, and Exploitation (MANE)

    + Triage critical incident reports and determine level, urgency, and scope of investigation

    + Interview staff, residents, and witnesses; gather and evaluate evidence; and write detailed, objective reports

    + Ensure timely and appropriate reporting to law enforcement and regulatory agencies

    + Audit medical and program records to assess compliance with ICF/IID regulations and internal policy

    + Monitor service sites to verify safe, effective, and rights-based care is being provided

    + Identify trends in incident data and propose systemic improvements to leadership

    + Deliver staff training on MANE prevention, resident rights, and mandated reporting protocols

    + Collaborate on quality assurance and performance improvement efforts across the agency

    + Travel to service sites and support other QA functions as needed

    + Work evenings and weekends as needed

     

    Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

    Education/Licensure/Certification/Experience:

    Experience Only:

    Six (6) years of relevant experience in an occupation related to the work assigned to this position

    OR

    Education and Experience:

    A combination of related education and/or relevant experience in an occupation related to the work assigned, such as working with a population with intellectual and developmental disabilities, equal to six (6) years.

     

    Substitutions

     

    Additional appropriate education will substitute for the required experience on a

     

    year-for-year basis

    Preferred Qualifications

    + Strong verbal and written communication skills, with the ability to tailor information to various audiences

    + Proven ability to multitask, manage time independently, and adapt to shifting priorities

    + Skilled in process development, problem-solving, and root cause analysis

    + Experience working with individuals with intellectual and developmental disabilities (IDD) and/or mental health needs

    + Knowledge of Immediate Care Facilities (ICF)/IID standards, survey processes, and quality service delivery for individuals with IDD

    + Proficient in software tools such as Google Workspace and UKG

    + Experience writing quality improvement plans and managing departmental functions

    + Flexible work hours and independent judgment

    + Labor Relations Alternatives (LRA) investigation certification, or ability to obtain within three months of hire

    Conditions of Employment:

    + Full background check: CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers) and reference check required

    + Must possess a valid, unrestricted U.S. Driver's License

    + Travel – Drive state car, van or bussette to various group homes, programming, admin & in community

    + TB test is required, requires Influenza and other vaccines as required by state and facility; must be 18 years or older.

    + Pre-employment Physical/Tests – Exert up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects

    + Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.

    Appeal Rights:

    If you receive notice that you have been eliminated from consideration for this position, you may file an appeal with the State Personnel Board or request a review by the State Personnel Director.
