About Grand Junction Regional Center:
Grand Junction Regional Center (GJRC) is a collection of group homes that serves adult individuals with intellectual and developmental disabilities (IDD) who need additional support to stabilize and return to community residential settings. GJRC's focus is on improving outcomes for individuals with IDD as well as promoting community integration. GJRC has a total of 10 homes nestled throughout established neighborhoods of Grand Valley. These homes feature warm, comfortable living spaces and private, fenced backyards with recreation equipment.
Description of Job
The Community Integration Coordinatoris responsible for designing, implementing, and evaluating individualized treatment plans that support adults with developmental disabilities in achieving meaningful engagement in work, leisure, and community life. This role combines clinical insight, program development, and community partnership to promote personal growth, independence, and inclusion.
Additionally, as a Community Integration Coordinator, you can expect to:
+ Conduct annual assessments to identify treatment needs in vocational, financial, recreational, and community living domains
+ Create customized learning and support plans that reflect individual strengths, preferences, and goals
+ Design and deliver training for direct care staff on individualized treatment methodologies and behavior support strategies
+ Coordinate and lead community-based activities and events, ensuring accessibility, transportation, and staff coverage
+ Serve as liaison to community organizations, including gyms, art centers, Special Olympics, and other recreational outlets
+ Audit personal bank accounts monthly for up to 30 individuals to ensure compliance with legal and agency standards
+ Collaborate with interdisciplinary teams to develop and monitor Individual Support and Service Plans (ISSPs)
+ Track outcomes, analyze data, and make adjustments to treatment approaches based on individual progress and staff feedback
+ Manage a designated program budget, including procurement of supplies, activity funding, and cost center reconciliation
+ Support agency initiatives by participating in meetings, trainings, and occasional direct care coverage as needed
+ Work some nights and weekends as needed

Education/Licensure/Certification/Experience:
Experience Only:
Five (5) years of relevant experience in an occupation related to the work assigned to this position
OR
Education and Experience:
A combination of related education and/or relevant experience in an occupation related to the work assigned, such as working with a population with intellectual and developmental disabilities, equal to five (5) years.
Substitutions
Additional appropriate education will substitute for the required experience on a
year-for-year basis
Special Qualifications
+ Complete and pass Safety Care training and QMAP training within the first 6 months of hire.

Conditions of Employment:
+ Full background check: CDHS employees (all Direct contact with vulnerable persons): CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers) and reference check required
+ Must possess a valid, unrestricted U.S. Driver's License
+ Travel – Drive state car, van or bussette to various group homes, programming, admin & in community
+ TB test is required, requires Influenza and other vaccines as required by state and facility; must be 18 years or older.
+ Pre-employment Physical/Tests – Exert up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects
+ Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
