    • About the Role

     

    We’re looking for a dedicated HIM Specialist to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you’ll be a vital part of our team, ensuring excellent service, collaboration, and patient outcomes in a fast-paced healthcare environment. The HIM Specialist is responsible for supporting the health information management processes within our healthcare system. This entry-level position focuses on the retrieval, preparation, scanning, and indexing of clinical documentation for the Albany Med Health System’s (AMHS) inpatient and outpatient facilities. The HIM Specialist ensures compliance with legal, regulatory, and organizational standards while maintaining the integrity and confidentiality of patient information.

     

    • What You’ll Do

    + Assist the HIM Operations Manager to ensure all clinical documentation is available to coding/billing and clinical staff via the electronic health record within 24 hours of discharge.

    + Retrieve and sort clinical documentation from departments when directed by HIM leadership team.

    + Prepare clinical documentation to be scanned by reconciling encounters, orders, referrals, etc. You'll follow up on incomplete record per departmental reconciliation guidelines.

    + Prep, scan and index incoming clinical documentation from internal providers and external referrers in accordance with departmental policies and procedures.

    + Perform thorough review of all incoming documents via mail, fax, email, etc. and redirect to appropriate departments accordingly.

    + Identify documents, which constitute the legal content of the patient's health record, in both electronic and paper formats to ensure integrity of record.

    + Assign appropriate document types and supporting identifiers such as date of service.

    + Assist in the collection and reporting of quality metrics related to PCMH and CMS quality measures by adhering to departmental policies that target appropriate mapping of the medical record (patient, encounter or order level).

    + Identify the contents of incoming paper and electronic clinical documentation to appropriately route documents to physicians, nurses, referral teams or other departments in accordance with department policies and procedures.

    + Properly complete destruction of records once prep, scan, index, and QA processing has been fulfilled. Understand departmental record retention schedule and assist with tasks related to storing, retention and purging patient records.

    + Meet productivity and quality benchmarks and scorecard expectations set by HIM Operations Manager.

    + Collaborate with the team to enhance data accuracy and integrity of medical records.

    + Notify HIM leadership of any workflow or build issues to mitigate potential problems as soon as possible.

    + Perform other tasks and projects assigned by HIM leadership team.

    + Take proactive steps to recognize active or potential security risks and report them to qualified IT staff or supervisor.

    + Adapt to the diversity of the customer to understand complex issues.

    • What You Bring

    + A high school diploma or GED equivalent and 1 or more years of experience in Health Information Management (HIM) or related work environment are required. In lieu of work experience, an associate degree in HIM with a minimum of 80 hours of practical experience in HIM in a hospital setting will be considered.

    + RHIA or RHIT certification preferred.

    + Familiarity with EHR systems (ideally EPIC and OnBase) and a basic understanding of Patient-Centered Medical Home (PCMH) and Centers for Medicare and Medicaid Services (CMS) quality measures is preferred.

    + Strong communication and teamwork skills.

    + Commitment to providing patient-first, high-quality service.
