Description
Join our mission-driven team to help individuals with intellectual and developmental disabilities receive the support they need to thrive.As a CAS Coordinator, you will play a critical role in ensuring the timely, accurate, and person-centered completion of the Coordinated Assessment System (CAS), supporting individuals, families, and staff throughout the process.
What You’ll Do:
+ Coordinate and Track Assessments
+ Proactively organize and maintain agency-wide CAS schedules and tracking.
+ Gather and prepare necessary documentation to ensure assessments are completed on time.
+ Communicate assessment dates and outcomes to staff and families clearly and supportively.
+ Facilitate CAS Implementation
+ Serve as the main contact for OPWDD assessors and field staff.
+ Provide accurate pre-assessment information and support individuals and families in understanding and preparing for assessments.
+ Quality Review and Integration
+ Review CAS results for accuracy and consistency with observed needs.
+ Collaborate with care managers and program teams to incorporate CAS findings into Life Plans and service delivery.
+ Training and Technical Support
+ Train and guide staff on CAS processes, documentation, and interpretation.
+ Help staff understand how CAS outcomes influence planning and funding.
+ Compliance and Reporting
+ Ensure processes comply with OPWDD regulations and agency policies.
+ Compile CAS data for audits, quality assurance, and strategic planning.
Requirements
+ 3+ years of experience working with individuals with intellectual and developmental disabilities
+ Education info here
+ Knowledge of OPWDD systems, services, and regulatory framework
+ Strong organizational and communication skills
+ Experience working with interdisciplinary teams
+ Detail-oriented and highly organized
+ Strong analytical and problem-solving abilities
+ Ability to build relationships with families, staff, and external partners
+ Commitment to person-centered and equitable service delivery
+ Proficient in Microsoft Office Suite and data management tools
+ Familiarity with Electronic Health Records (EHR), TABS, or CHOICES
+ Valid NYS Drivers’ License and reliable transportation to travel to various program locations.
Preferred Qualifications/Skills:
+ Experience with OPWDD’s CAS tool or DDP-2
+ Coordination experience
Salary Description
33.00 hr
