Job Description:
The Clinic Care Guide serves as a healthcare navigator for identified patients and their caregivers. This role is responsible for serving as a single point of contact for patients, providing outreach and supporting patients with education around preventative care and chronic disease management, referral management and transitions of care. The embedded/centralized care guide leads the daily team huddle and engages in pre-visit, visit and post visit planning to support the health care team in care gap closure, care navigation and care plan outcome
attainment to efficiently deliver proactive, patient-centered care. This position works collaboratively with the care management team, patients, caregivers, healthcare providers, payers, community-based providers, and other involved parties to provide effective, efficient, value-based care services.

Essential** **Functions
+ Serves as a primary point of contact between the patient and the care team to anticipate and appropriately address patient questions and concerns.
+ Establish trusting, supportive, collaborative relationships with patients and their clinic team.
+ Conduct new patient orientation providing education and resources available through their health plan.
+ Assist with coordination of care:
+ Overcoming barriers to follow up appointments
+ Making specialist appointments
+ Obtaining medical records from specialists, previous primary care physicians and hospitals.
+ Monitor hospitalized patients and their transition of care to help keep the care team informed.
+ Engage in pre-visit planning to surface important information to close gaps in patient care.
+ Demonstrate knowledge of HIPAA regulations and maintain the confidentiality of patient information to be compliant with internal policies and procedures.
Minimum Qualifications
+ Two years of experience navigating a complex organizational system (e.g., college/university studies, military service, healthcare organization, federal programs).
+ Two years of work experience in healthcare
+ Strong Interpersonal, written, and verbal communication skills.
+ Experience working in a collaborative, team-oriented environment.
+ Patient-centered attitude and ability to work with colleagues in a professional and courteous manner.
+ Strong computer skills and willingness to continuously improve them: navigating reporting platforms, managing data spreadsheets, wordprocessing, e-mail and the internet
Preferred Qualifications
+ Two years of college healthcare related coursework
+ Bilingual in English and Spanish
+ Knowledge of medical and pharmacological terminology, CPT, HCPCS, ICD coding
Physical Requirements:
Physical Requirements
+ Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
+ Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
+ Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
+ Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
+ Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
+ May be expected to stand in a stationary position for an extended period of time.
+ For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
