    Now Hiring: Licensed Practical Nurse (LPN) | Community-Focused Primary Care

     

    Full-Time | Monday–Friday, 8 AM–5 PM

    Your Role: Licensed Practical Nurse (LPN)

    As a key member of our Primary Care team, you’ll provide essential nursing care, support chronic disease management, and help coordinate referrals and specialist services. You’ll work closely with a multidisciplinary team to ensure patients receive the comprehensive, compassionate care they deserve.

    What You’ll Do

    + Deliver routine nursing care in a primary care setting.

    + Support care coordination for high-risk and chronically ill patients (e.g., Diabetes, HIV, Hypertension, Obesity).

    + Assist with referrals and follow-up care, ensuring seamless patient experiences.

    + Participate in women’s health initiatives and conduct medical assessments.

    + Accurately monitor and document vital signs and patient progress.

    + Travel between Greenwich and Stamford locations as needed.

    What You Bring

    + Licensure: Active LPN license in the state of Connecticut.

    + Experience: 3+ years as an LPN, with a strong background in chronic care or high-risk patient management.

    + Tech-Savvy: Comfortable using Electronic Health Records (EHR).

    + Languages: Bilingual in Spanish or Haitian Creole is a strong plus.

    + Team Player: Collaborative, communicative, and compassionate.

    + Preferred: 2+ years in a primary care or outpatient setting.

    + Knowledge of population health and care coordination.

    + Strong clinical judgment and attention to detail.

    + Ability to multitask and manage time effectively.

    + A patient-first mindset and a passion for community health.

    Pay and Benefits

    The pay range for this position is $30.00 - $33.00/hr.
