    Job Summary

    The Diabetes Peer knows their community well and acts as a cultural mediator between their church or faith community and the Baylor Scott & White Health system. They work closely with Community Health and Diabetes Educators. They provide basic diabetes and health education, informal counseling, and self-management support that fits their specific cultural setting.

    Essential Functions of the Role

    + Assists in tailoring a faith-based diabetes program relevant to the Diabetes Peer's community beliefs.

    + Promotes culturally competent care by educating DHWI staff about beliefs and practices in their faith community.

    + Recruits fellow members of their faith community who have diabetes to participate in a diabetes education program.

    + Conducts a comprehensive diabetes education program and evaluate the achievement of learning goals of each participant.

    + Partners with Community Health and Diabetes Educators in helping prescribed therapies.

    + Links people with appropriate community resources and services available from DHWI.

    + Attends 160 CHW training course provided by DHWI.

    Key Success Factors

    + General knowledge of diabetes care modalities, therapies, terminology and equipment.

    + Must be able to communicate thoughts clearly; both verbally and in writing.

    + Ability to effectively use a variety of teaching methods.

    + Cultural proficiency related to health practices

    + Pledged, dependable, adjustable, inventive, resourceful and has an exceptional desire to help the community.

    + Able to interact positively with patients as needed.

    + General computer skills include using software applications, data entry, information security, electronic medical documentation, handheld scanning, and email.

    QUALIFICATIONS

    + EDUCATION - H.S. Diploma/GED Equivalent

    + EXPERIENCE - Less than 1 Year of Experience
