Shift: Evening shifts available (4 PM–12 AM)

We’re currently seeking dedicated LPNs and RNs to join our evening shift team, providing one-on-one care to children and young adults with specialized medical needs in the comfort of their homes.

What You’ll Be Doing:

You’ll be a consistent, compassionate presence during a critical time of day for our clients and their families. Responsibilities may include:

+ Administering evening medications and treatments

+ Providing g-tube, trach, or ventilator support

+ Monitoring vital signs and general condition

+ Documenting care provided and changes in condition via electronic charting

+ Communicating with families and care team members as needed

+ Following the physician’s plan of care with attention and compassion

You’ll work independently in the client’s home—but never without support. Our nurse supervisors and 24/7 on-call team are always just a phone call away.
