+ Position Summary
+ Under general supervision of Clinical Services Manager; provides professional service in processing referrals and coordinating fiscal and care arrangements for neonatal through geriatric clients, performs services to walk-in clients, assists with coordination of staff caseload and scheduling, assists with orientation of staff and assists with Quality Assurance activities. Ensures all actions taken in carrying out responsibilities support patient centered care.

+ Receives telephone or interview referrals from various sources; physicians, clients, families, social workers, discharge planners or community persons and processes intake in computer.
+ Acts as resource person, keeping current of agency programs, insurance carriers and funding sources.
+ Assesses information from referral sources pertinent to patient admission. Interprets agency services, admission criteria and medical policies to referral sources.
+ Coordinates appropriate services by obtaining orders for any additional services/equipment at time of intake.
+ Obtains or schedules the Face-to-Face physician encounter dependant on funding.
+ Inputs diagnosis coding for home health reimbursement and assists staff when appropriate.
+ Contacts insurance companies with clinical updates and negotiates an ongoing service schedule and communicates information to Case Manager.
+ Provides clinical services to walk-in clients
+ Maintains competency in requirements for care delivery and documentation of infusion services per various contracts held with vendors of infusion services. Negotiates rates within predetermined guidelines.
+ Coordinates caseloads by assisting Clinical manager in assigning case managers to clients.
+ Qualifications, Knowledge & Experience
+ Required Qualifications (Including any licensure, certification, education):
+ Licensed as a Registered Nurse or LPN and authorized by the State of Iowa to practice nursing.
+ Graduation from a school of nursing.
+ Achievement of Baccalaureate degree in nursing within 6 years of hire if hired after January 1, 2012.
+ Certification in HCS-D Homecare Coding Specialist Diagnosis; must be completed within 12 months of hire.
+ Possession of a current valid driver’s license and must maintain eligibility for coverage under Mary Greeley Medical Center’s insurance policy.
+ Organizational Requirements:
+ Maintain stroke education per regulatory requirements.
+ Mandatory Reporter training or within 90 days of hire and must maintain throughout employment.
+ Preferred Qualifications:
+ Professional certification in nursing specialty.
+ Required Knowledge, Skills & Experience:
+ Two years experience in medical surgical nursing, home care, or hospice.
+ Skill in effective oral and written communication in English. Ability to explain procedures and give information. Ability to report abnormal conditions appropriately.
+ Proficient in communication skills.
+ Ability to pursue and foster a sense of team and partnership across all disciplines.
