Home Health Aide – Hospice
**Location:** Vancouver, WA
**Schedule:** Full Time | Day Shift | 1.0 FTE
**Hourly Range:** $22.04 – $33.10 (DOE, union-represented)

Join PeaceHealth’s compassionate hospice care team and make a meaningful difference in the lives of patients and families facing life-limiting illness. We are currently hiring a **Home Health Aide – Hospice** to provide vital support and personal care to patients in the comfort of their homes.
In this role, you’ll be responsible for assisting patients with daily living activities, providing basic nursing care under the supervision of a registered nurse, and supporting safe mobility and comfort at home. As a valued member of the interdisciplinary care team, you’ll play a key role in ensuring each patient receives respectful, dignified, and holistic care.
Key Responsibilities:
+ Assist with personal care tasks such as bathing, grooming, dressing, and toileting
+ Support patients with mobility, transfers, exercises, and ambulation
+ Measure and record vital signs, intake/output, and perform other delegated tasks
+ Provide light housekeeping, meal preparation, and linen changes per care plan
+ Accurately document care and promptly report changes in condition to the RN
+ Collaborate with nurses, therapists, and family caregivers to ensure coordinated care
Qualifications:
+ Certified Nursing Assistant (CNA) in Washington (required)
+ Minimum 2 years of CNA and/or home health aide experience in a clinical setting
+ Valid driver's license, proof of auto insurance, and reliable transportation
+ Current Basic Life Support (BLS) certification
+ Food Handler’s Card (required at hire)
