Are you a compassionate and experienced Registered Nurse (RN) with a passion for guiding patients through their healthcare journey? Novant Health is seeking a dedicated Clinical Services Navigator to join our Cardiac Surgery Clinic team. In this pivotal role, you'll serve as the primary liaison between patients and their medical teams, ensuring seamless coordination of care and exceptional patient experiences.
Key Responsibilities:
+ Atrial Fibrillation Clinic Coordinator is to be responsible for the coordination of clinical services at the population level for patients of the Atrial Fibrillation Clinic.
+ Care Coordination: Collaborate with physicians, case managers, and other healthcare professionals to facilitate smooth transitions from acute to ambulatory care settings.
+ Patient Advocacy: Act as a patient advocate, providing support and guidance throughout the healthcare process.
+ Maintain current knowledge of available community resources, post-acute care, and ambulatory options to assist patients effectively.
+ Organize and arrange educational sessions, such as atrial fibrillation (AF) talks, with primary care groups to enhance patient understanding and engagement.
+ Create and manage a database of AF Clinic patients for quality assurance and to guide clinic directions.
+ The position will manage the AFC population and be tasked with growing that population appropriately within our footprint.
+ Coordinate with procedural and clinic schedulers, nursing staff, AFC clinicians (PAs and NPs), and supervising electrophysiologists to ensure that each patient's established AF treatment plan is fulfilled
Qualifications:
+ Education: Bachelor's degree preferred in Nursing, Social Work, Healthcare Administration, or a related field.
+ Licensure: Active RN licensure required.
+ Experience: Minimum of three to five years of nursing experience required; two years of experience in care of targeted patient population preferred.
+ Skills: Strong collaboration skills with a multidisciplinary healthcare team; in-depth knowledge of community resources, post-acute care, and ambulatory options; commitment to Novant Health's values of respect, compassion, inclusion, empowerment, inspiration, and courage.

- C ardiology background, preferably electrophysiology (cardiac devices).

Education
4 Year / Bachelors Degree Preferred
Relevant fields of study include Nursing, Social Work, or Healthcare Administration.
Licensure/Certification: Active RN licensure Required
