    This position is responsible for arranging for and coordinating community health care services for patients being released from DOC facilities. The position is responsible for assessing post discharge needs including but not limited to home care, nursing home placement or other structured placement due to disabling condition, durable medical equipment, financial assistance, counseling, and other community resources as deemed necessary.

     

    Position has direct contact is Incarcerated Persons (IP).

    Minimum Qualifications

    To be qualified for and considered for this position, applicants must meet all the minimum qualifications listed below. Ensure that your resume contains descriptions of your work experiences sufficient for comparison against the requirements stated below and indicate the beginning and ending month and year for each job held.

    Licensure/Experience

    To qualify, you must have current licensure in Minnesota to practice Social Work and meet **one** of the following:

    + Licensed Social Worker (LSW) and a minimum of three years professional social work experience; OR

    + Licensed Independent Social Worker (LISW) and a minimum of 18 months of professional social work experience; OR

    + Licensed Graduate Social Worker (LGSW) and a minimum of 18 months of professional social work experience; OR

    + Licensed Independent Clinical Social Worker (LICSW) and a minimum of 18 months of professional social work experience.

     

    Knowledge, Skills, and Abilities

    In addition to the above, you must meet all the following knowledge, skills, and abilities:

    Knowledge of:

    + Individual, family, and group process intervention techniques sufficient to develop and evaluate complex treatment interventions for specialized client populations.

    + Theories and methods of psycho-social assessment to evaluate the social service needs of the client and provide recommendations to the treatment team.

    Skills in:

    + Communication and working collaboratively with others to address the needs and concerns of clients.

    + Recognizing the importance of quality service to families, clients, staff, physicians, peers and outside contacts.

    + Problem solving sufficient to develop and evaluate complex treatment intervention for specialized client populations.

    Ability to:

    + Determine differential treatment objectives for clients with complex needs.

    + Recognize the personal dignity of patients, visitors, and others in responding to expectations, complaints and requests.

    + Must have a valid driver's license.

    Preferred Qualifications

    + Three or more years of professional social work experience.

    + Licensure as a LISW, LGSW, or LICSW.

    + Experience in client discharge planning in the areas of home care, nursing home placement, durable medical equipment, financial assistance, counseling, and/or other community resources.

    + Experience working with correctional clients.

    Physical Requirements

    Additional Requirements

    The selection process may include screening the top candidate(s) on their criminal history, predatory offender registration, offender associations and/or other applicable background information in accordance with Department of Corrections’ policy. Top candidate(s) must pass a criminal history check which includes successful completion of all sentencing and/or restitution requirements of law enforcement jurisdictions, including probation.

     

    Employment history/reference checks, including incidents of sexual harassment, will also be conducted on the top candidate(s) in accordance with Department of Corrections’ policy. This may include a review of job performance and discipline records.

     

    New employees will be fingerprinted within the first 30 days of employment.
