Position Overview

TheMDS Nurse Assessoris responsible for collecting and submitting assessment data critical to the health and well-being of our residents. You will support the MDS/PPS Coordinator in completing PPS and/or OBRA assessments for both subacute and long-term care residents, as well as perform additional tasks as assigned.

Responsibilities

RN - MDS Nurse Assessor - Essential Job Functions:

+ Accurately and timely complete and submit Minimum Data Set (MDS) assessments

+ Ensure compliance with Federal, State, and facility policies

+ Actively participate in utilization management processes, including PDPM and state case mix systems

+ Conduct resident interviews

+ Compile, monitor, and audit clinical documentation

+ Communicate clinical issues effectively to appropriate clinicians

+ Support and assist the MDS/PPS Coordinator as needed

+ Demonstrate thorough knowledge of policies and procedures

+ Utilize electronic timekeeping systems as directed

+ Follow facility dress code and identification policies

+ Abide by all policies related to electronic devices, computers, and tablets

+ Uphold Elderwood’s Mission, Vision, and Values

+ Maintain compliance with company code of conduct, HIPAA, and regulatory standards

+ Perform other duties as assigned by supervisor or Clinical Reimbursement Department

Qualifications

RN - MDS Nurse Assessor - Qualifications:

+ Licensed Professional Nurse (RN licensure preferred) with current state license

+ Prior experience with MDS 3.0 preferred

+ Long-term care experience preferred

+ Excellent verbal and written communication skills

+ Self-motivated, highly organized, and dedicated

+ Willingness to train the right candidate
