    Description

    Position Summary:

    The Resource Navigator is part of the prevention team at WestCare TN is an integral member of the prevention team responsible for connecting participants and staff with up-to-date, comprehensive local resources. Working closely with the Outreach Coordinator, the Resource Navigator ensures the program has current access to public health care, government assistance, and community services that support holistic health and wellness.This position serves Cocke County only.

    Essential Job Functions:

    + Collaborate with the Outreach Coordinator to identify and maintain connections with local healthcare providers and community organizations.

    + Maintain and update a detailed database of healthcare and support services including alternative pain management, pain management clinics, massage therapy, telehealth providers, and programs addressing physical, mental, and spiritual wellness.

    + Research and verify availability and eligibility criteria of services to ensure participants have access to relevant resources.

    + Provide resource information to program staff and participants to facilitate appropriate referrals.

    + Support outreach efforts by supplying resource information for workshops, educational materials, and community engagement activities.

    + Track and report resource utilization and gaps to inform program planning and improvements.

    + Establish and nurture relationships with public health, government, and community partners.

    + Maintain accurate records and documentation related to resource mapping and navigation activities.

    + Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation.

    + Collaborate with other WestCare Tennessee team members to promote and meet requirements.

    + Participate in

    + Will engage and complete the required WestCare Tennessee eLearning, and any other program related trainings.

    + Perform other duties assigned by the Project Coordinator/ Director

    Qualifications

    Essential Qualifications:

    + Associate’s degree in Public Health, Social Work, Health Education, or related field is required.

    + Experience in community resource navigation, case management, or public health outreach.

    + Strong research and organizational skills with attention to detail.

    + Excellent communication and interpersonal skills.

    + Familiarity with healthcare systems, community resources, and public assistance programs.

    + Ability to work independently and collaboratively within a team.

    + Proficiency with databases, Microsoft Office, and other data management tools.

    Preferred:

    + Knowledge of alternative pain management and integrative health services.

    + Experience working within Tennessee or similar community health environments.

    Working Conditions:

    + Work is primarily performed in an office or indoor setting.

    + Some outdoor activities and travel willbe required.

    + Some travel will be required for training and conferences.

    + Regular and prompt attendance is required throughout employment.

     

    Essential Physical and Mental Demands of the Job

    The employee must be able to perform the following essential duties and activities with or without accommodation:

    Physical Demands:

    + Requires mobility and physical activity: Having an adequate range of body motion and mobility to work in an office or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended periods of time, bending, twisting, reaching, balancing, occasional lifting and carrying of up to 50 pounds. Use of computer and telephone systems is required which includes coordination of eye and hand, and proper utilization by hand (typing, writing, and working with files).

    + Requires talking: Expressing or exchanging ideas by means of the spoken word. Talking is required to impart oral information to employees, the public, and in those activities in which the employee is required to convey detailed or important spoken instructions to others accurately, loudly, or promptly.

    + Requires hearing: Hearing is required to receive and communicate detailed information through oral communication.

    + Requires seeing: Clarity of vision including proper eyewear at 20 inches or less and at distance.

    + The normal work routine involves no exposure to human blood, body fluids or tissues. However, exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee.

    Mental Demands :

    + Requires the ability to collect and analyze complex numerical and written data and verbal information to reach logical conclusions.

    + Requires the ability to work and cooperate with co-workers, managers and the public at all levels in order to exchange ideas, information, instructions and opinions.
