    Description

    Position Summary:

    Community Health Worker (CHW) is part of the prevention team to serve as a vital link between participants and healthcare services under the Tennessee Health Resiliency Grant. CHW will ensure coordinated support for each participant by conducting health screenings and assessments, advocating for their healthcare needs, coordinating services, and facilitating educational workshops. This role also involves fostering participant engagement through regular meetings and leading monthly integrated treatment team meetings.This position serves Cocke County only.

    Essential Job Functions:

    + Conduct health screenings and comprehensive assessments for program participants.

    + Coordinate individualized service plans to address participants’ unique healthcare and social needs.

    + Advocate on behalf of participants to ensure access to appropriate healthcare and community resources.

    + Facilitate workshops and educational sessions to promote health resiliency and wellness.

    + Meet regularly with participants to provide ongoing support, monitor progress, and adjust care plans as needed.

    + Lead and facilitate monthly integrated treatment team meetings involving healthcare providers, social services, and other stakeholders to promote collaborative care.

    + Maintain accurate and timely documentation of participant interactions, services provided, and outcomes.

    + Collaborate closely with community partners, healthcare providers, and program staff to ensure seamless service delivery.

    + Support data collection and reporting to meet grant requirements.

    + Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation.

    + Collaborate with other WestCare Tennessee team members to promote and meet requirements.

    + Participate in

    + Will engage and complete the required WestCare Tennessee eLearning, and any other program related trainings.

    + Perform other duties assigned by the Project Coordinator/ Director

    Qualifications

    Essential Qualifications:

    + Bachelor’s degree in Health, Social Work, or related field is required.

    + Previous experience as a Community Health Worker or in a similar healthcare or social service role.

    + Strong interpersonal and communication skills with the ability to build trust and rapport with diverse populations.

    + Knowledge of health screening tools, care coordination, and patient advocacy.

    + Ability to facilitate group workshops and meetings effectively.

    + Comfortable working both independently and as part of a multidisciplinary team.

    + Proficiency with basic computer skills and data entry.

    + Commitment to cultural competency, health equity, and community empowerment.

    Preferred:

    + Experience working in Tennessee or similar communities.

    + Familiarity with trauma-informed care and integrated treatment models.

    Working Conditions:

    + Work is primarily performed in an office or indoor setting.

    + Some outdoor activities and travel willbe required.

    + Some travel will be required for training and conferences.

    + Regular and prompt attendance is required throughout employment.

     

    Essential Physical and Mental Demands of the Job

    The employee must be able to perform the following essential duties and activities with or without accommodation:

    Physical Demands:

    + Requires mobility and physical activity: Having an adequate range of body motion and mobility to work in an office or outdoor environment including standing and walking (even and uneven surfaces), sitting for extended periods of time, bending, twisting, reaching, balancing, occasional lifting and carrying of up to 50 pounds. Use of computer and telephone systems is required which includes coordination of eye and hand, and proper utilization by hand (typing, writing, and working with files).

    + Requires talking: Expressing or exchanging ideas by means of the spoken word. Talking is required to impart oral information to employees, the public, and in those activities in which the employee is required to convey detailed or important spoken instructions to others accurately, loudly, or promptly.

    + Requires hearing: Hearing is required to receive and communicate detailed information through oral communication.

    + Requires seeing: Clarity of vision including proper eyewear at 20 inches or less and at distance.

    + The normal work routine involves no exposure to human blood, body fluids or tissues. However, exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee.

    Mental Demands :

    + Requires the ability to collect and analyze complex numerical and written data and verbal information to reach logical conclusions.

    + Requires the ability to work and cooperate with co-workers, managers and the public at all levels in order to exchange ideas, information, instructions and opinions.
