UW Medical Center - Montlake** Transplant has an outstanding opportunity for a **Nurse Practitioner or Physician Assistant (ARNP/PA) to join our KIDNEY Transplant Team.** **WORK SCHEDULE
40 hours per week
Day Shift
Monday-Friday
POSITION HIGHLIGHTS
The Advanced Practice Provider will provide specialized care for transplant patients and family assessment for Kidney care. This is a great opportunity to join our interdisciplinary collaborative team of outstanding providers.
PRIMARY JOB RESPONSIBILITIES
+ Conduct initial consults, as well as follow-up visits for patients.
+ With keen attention to the specific needs of patients, deliver excellent care, with compassion, motivation, and sound medical knowledge.
+ Coordinate and monitor patients' status in conjunction with attending physicians, social workers, and other providers.
+ Respond to patients' healthcare needs, providing triage, urgent care and interventions as necessary/appropriate.
+ Follow patients, including longitudinal care and follow-up, in conjunction with other providers. **REQUIREMENTS**
+ Nationally certified ARNP/PA-C with DEA.
+ Current and valid Washington State Registered Nurse Licensure and ARNP Licensure or Physician Assistant License.
+ Graduate Degree from an accredited Nurse Practitioner Program or graduate of an accredited PA program.
+ Proven excellence working in an interdisciplinary collaborative team environment.
+ Effective interpersonal skills, leadership skills, and clinical expertise in patient care.
