Position Summary: The Children's Nurse Practitioner with an affiliation agreement of a psychiatrist, PROVIDES services within the scope of his/her license/certification and according to the professional standards of his/her discipline. This position will have the responsibility of Providing psychiatric care including assessing and providing ongoing medication management, as needed, to children and adolescents within the MHOTRS Clinic.
Essential Accountabilities: This does not exclude the assignment of duties or exclude the performance of other duties, not mentioned, as assigned by supervisors.
ACCOUNTABILITIES:
+ Conduct comprehensive psychiatric evaluations and diagnostic assessments for children and adolescents.
+ Develop, implement, and monitor individualized treatment plans in collaboration with the clinical team.
+ Prescribe and manage psychotropic medications, monitor side effects, and adjust treatment as clinically indicated.
+ Participate in emergency consultation services and provide recommendations, treatment plans and crisis interventions.
+ In conjunction with the Physician and/or Medical Director/Assistant Medical Director develop and maintain a network of referral sources for primary and specialty care for use as needed to address medical referrals from the program.
+ Review lab and other medical reports and take actions as appropriate, i.e. contact primary care provider, to ensure appropriate follow up.
+ Provide psychoeducation to patients and families about diagnoses, medications, and treatment options
+ Collaborate with therapists, case managers, and other clinical staff to ensure coordinated care.
+ Meet the treatment team on a regular basis to keep them current regarding treatment plan and be apprised of any issues that may affect it.
+ Maintain thorough, accurate, and timely clinical documentation in accordance with OMH regulatory standards.
+ Participate in clinical team meetings, case conferences, and ongoing quality improvement initiatives.
+ Ensure care provided aligns with current best practices, ethical standards, and state/federal regulations.
+ Consult with affiliated psychiatrist and/or primary care /specialty physician re: medical services and/or diagnosis/treatment referrals that he/she provides.
+ Monitor client's psychiatric needs through direct observation and when necessary make appropriate referrals for psychiatric hospitalization or other levels of care.
+ Monitor treatment provided by collateral(s)/other team members by observing them for the purpose of giving instructions, providing feedback and adjusting practices.
+ Participate and attend meetings/ trainings at various sites.
+ Participate in OMH approved orientations and trainings.
+ As requested, provide training to clinical staff, i.e.: medication management, health care maintenance and/or special topics of interest
+ Must be able to communicate information verbally and in writing for the purposes of giving instructions, providing feedback and adjusting work practices.
+ Maintain computer literacy; E H R platform, order connect etc.
+ Maintain confidentiality at all times.
+ Represent agency and clinical services at inter-agency meetings and conferences.
+ Participate in all audit processes, including preparation of Plan of Corrective Action (POCA), if necessary.
Job Qualifications
+ Licensed and registered as a Nurse Practitioner in New York State is required.
+ 1–2 years of experience providing psychiatric care to children and adolescents, preferably in an outpatient or community mental health setting.
+ Familiarity with MHOTRS regulations and service delivery models a plus
+ Current DEA registration and NPI number.
+ CPR/BLS certification.
