Job Summary and Qualifications
The Population Health Navigator (PHN) is accountable for assuring a high standard of quality and coordination of care, in support of HealthONE value-based care (VBC) programs with payers in the market. The PHN has a defined role, working in an advisory/coaching capacity to support the CCP practices and assist in creating and monitoring standards of care and optimization of performance from value-based contracts. The PHN utilizes an evidence-based approach to facilitate QI methodology and tools within practices, including baseline assessments, reporting measures and facilitating meetings with practice specific QI teams working towards achieving measurable outcomes.
DUTIES INCLUDE BUT NOT LIMITED TO:
+ Serve as a key contact for the Population Health Navigation team specific to daily assignments, workflows, monthly agendas, and guidance on provider practice approach for successful practice transformation
+ Focus on creation of team workflows, best practices and guidance documents to support standardization among the team practices and interventions
+ Liaise directly with the Director of Population Health to discuss PHN team initiatives, and support team communication with transformation outreach efforts
+ Manage above in addition to assigned provider sites as Population Health Navigator contact to those practices directly assigned
+ Provide onsite and telephonic practice visit coaching using a standardized approach and demonstrates the ability to effectively lead and facilitate improvement teams towards measurable outcomes.
+ Maintain a supportive and collegial role with the primary care team and collaborates to meet provider and practice needs.
+ Conduct practice needs assessment in order to learn more about practice demographics, their daily operations and current workflows.
+ Support the efforts of providers and practice teams on office-based care delivery interventions resulting in cost of care savings and improved health outcomes for patients.
+ Shadow, educate and/or train physicians and medical staff as necessary in clinical workflows and coordination of care to improve clinical quality and achieve VBC goals.
+ Obtain and analyze quality metrics and reports for care opportunities, supports practice implementation of care coordination and care management, identifies action plans for providers to implement to improve cost, quality and the patient experience and participates in design, development and implementation of community learning forums.
+ Provide education for practices to develop expertise with metrics and data review for quality improvement.
+ Support new team members in pororocas for report building and data pulls from internal data analytics engine for support to the practice sites
+ Collaborate with the Director of Population Health and CCP leadership on communication to physicians regarding available patient care programs through the VBC programs- both written/email communication and visits to provider meetings as necessary.
+ Review reports from payers and assist management in the interpretation of the reports and actions needed, including project initiatives using PDSA methodology.
+ Test and pilot new VBC initiatives and work with CCP leadership on refining processes, helping achieve contractual metrics.
+ Assist in the development, implementation and monitoring of corporate and/or HealthONE policies and procedures as they relate to VBC programs.
+ When the need arises, we will perform other duties as assigned by the supervisor.

This is an office-based position with the opportunity to work remotely at times or as approved by the Director of Population Health. Travel is required to make onsite visits to the practices and attend educational sessions.
