    Job Summary

    The Community Outreach Program Coordinator assists the Member Engagement Program Supervisor in coordinating multiple programs. This position helps members and community partners understand, navigate and/or connect with enrollment assistance at Oregon Health Plan (OHP) and Medicare ‘pop up’ locations such as food banks, homeless shelters and community events. The role focuses on outreach and engagement activities with CareOregon members and community partners including finding new locations for connecting with potential members, planning recurring events at the same locations, and coordinating with OHA Community Assistors to help with enrollment. The goals of the position include establishing and maintaining positive CareOregon relationships within the community while promoting member retention and engagement strategies.

    Essential Responsibilities

    Program Coordination

     

    + Develop meaningful partnerships and relationships with community organizations to identify new outreach sites within assigned county(s).

    + Collect, input and maintain membership data, engagement, eligibility and participation statistics by creating reports and data that show member interaction with programs.

    + Coordinate logistics for community outreach events and community presentations.

    + Maintain and track membership data, engagement, eligibility, and participation statistics by creating reports and data that show member interaction with programs and the associated cost.

    + Create and maintain the monthly outreach program calendars and update flyers, posters, health fair/expo materials and other outreach materials with the Brand, Marketing and Communication department, to promote activities.

    + Maintain confidentiality for all member customers, ensure compliance standards are met, and consistently review procedures for HIPAA and other legal compliance.

    + Utilize CareOregon systems such as QNXT, as well as state databases, such as MMIS and PH Tech, to determine member eligibility for OHP/Medicare.

    + Provide information regarding outreach events and activities for marketing purposes.

    + Facilitate enrollment in Medicaid/Medicare by referring members to partner OHP assisters and agents.

    + Assist CareOregon Member Engagement Program Supervisor in logistics, planning and other duties for new and emerging projects as needed.

    + Support CareOregon Community Advisory Board Meetings as needed.

    + Actively identify and recruit suitable members to the CareOregon Community Advisory Board (COCAB) while at community events.

     

    Navigation

     

    + Coordinate with Population Health Partnerships (PHP) caseworkers, community partners, providers and members.

    + Convey routine and complex information regarding health plans and their systems to clients, providers and community partners; easily navigate DHS, OHA and other social services.

    + Assist members and the public with navigating information regarding Social Security, Social Security Disability, DHS, and OHP benefit structures.

    + Participate in CareOregon customer service training.

    + Support community impact sponsored events.

    Experience and/or Education

    Required

    + Minimum 3 years’ administrative or community support experience in a healthcare and/or social services environment

    + Complete and maintain Community Health Worker Training within 12 months in the position

    + Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle also required

     

    Preferred

     

    + Oregon Health Agent Medicare License

    + Oregon Health Plan Enrollment Assister Training Certification

    + Experience providing Oregon Health Plan enrollment assistance

    + Formal education in public health or social work (AA+)

    + Completed Community Health Worker Training

     

    Knowledge, Skills and Abilities Required

     

    Knowledge

     

    + Knowledge of basic managed care concepts and the OHP

    + Knowledge of the CareOregon Medicare benefits

     

    Skills and Abilities

     

    + Understanding of Social Security, Social Security Disability, and Department of Human Services benefit structures or ability to learn

    + Ability to conduct basic research to identify and evaluate potential opportunities

    + Intermediate computer skills in MS Office Suite and Outlook

    + Strong recordkeeping skills

    + Excellent written and oral communication skills and the ability to assist with all forms of correspondence

    + Ability to effectively interact with individuals at all levels, both internal and external to the organization
