Acute Care Clinical Pharmacist
Full-time, 40hours/week. Majority 1st shift with rotating 2nd shift requirement and every 3rd weekend
Novant Health is seeking a Clinical Pharmacist, to join our hospital Pharmacy Float Pool, to deliver optimal medication management across all dimensions of care, every time. Float Pool team members will support the facilities in the Winston Salem Market including Forsyth Medical Center, Thomasville Medical Center, Clemmons Medical Center, Kernersville Medical Center and Medical Park Hospital. This individual will need the ability to travel to all facilities in the market as scheduled.

Essential functions include:
+ Facilitates pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence- based best practices and regulatory standards
+ Completes critical patient monitoring and reviews patient profile/ chart to identify, prevent, or mitigate drug- related problems,
+ Partners as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient- centered care
+ Makes appropriate evidence-based, patient- centered medication recommendations
+ Participates in the management of medical emergencies
+ Provides discharge medication review, reconciliation, and counseling as appropriate
+ Maintains proficiency in and actively engages in central dispensing functions and operational programs
+ Oversees pharmacy operations and technician practice/activities
+ Maintains proficiency in preceptor roles and actively emulates education/ mentorship skills

Qualifications
+ Education:4 Year/Bachelor's Degree in Pharmacy, required. Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification, required. Doctorate of Pharmacy, preferred.
+ Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
+ Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
+ Additional skills required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team
