    Responsibilities

    + Treatent: Provides individual, family and/or group outpatient therapy in a collaborative environment within a medical practice. Implements treatment plan using best practice modalities and interventions to obtain optimal patient outcomes. Evaluates crisis situations and applies appropriate interventions. Completes required patient clinical assessments determining level of care and treatment interventions using current diagnostic guidelines. Maintains ongoing follow-up care and patient engagement as needed. Assists patients in achieving overall health goals. Refers patients to higher level of care or request consultations as needed. Complies with required standards for medical/legal reporting situations. Reviews patient plan of care.

    + Medication Management: Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring under direction of supervising physician

    + Tracking and Documentation: Systematically tracks treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. Tracks patient follow up and clinical outcomes using a registry. Documents in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.

    + Behavioral Health: Knowledge of psychiatric medications, psychiatric diagnosis, mental health concepts, and evidence based therapeutic interventions and treatments. Ability to establish and maintain control of violent patients, defend self and others against violent assaults, ability to react appropriately during a psychiatric crisis. Ability to intervene in a therapeutic manner for behavioral patterns demonstrated by patients with a mental illness, personality disorder, abusing substances or developmental disabilities.

    Qualifications

    + Education: Graduate Degree, required. Master’s Degree in Psychology, Social Work, Counseling or related field, required.

    + Experience: Minimum of three yrs experience, required.

    + Licensure/Certification: LPC, LCSW, Clinical Psychologist or PMHCNS (North Carolina), required.

    + Additional Skills (required): Good working knowledge of computers, Microsoft Office and medical-based software. Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions. Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions. Should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills. Good knowledge of psycho-pharmacology. Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.

    + Additional Skills (preferred): Non-Violent Crisis Intervention certification and all other duties as assigned.
