Summary This position serves as Senior Licensed Professional Mental Health Counselor (LPMHC) in the Mental Health Residential Recovery and Treatment Program (MHRRTP) at the Alexandria VA Health Care System (AVAHCS) main campus. Responsibilities Senior Licensed Professional Mental Health Counselor in MHRRTP works closely with an interdisciplinary team in coordinating care for the veteran. The incumbent completes mental health psychosocial assessments, treatment plans and discharge planning. They provide supportive services to the family and caregiver as appropriate and make referrals to other clinics or programs at the VA and/or community agencies. The incumbent will assist with short-term and long-term interventions to enhance compliance with treatment and programming, provide individual and group evidence-based psychotherapy services, crisis intervention and promotion of whole-health principles. Primary responsibilities include evaluation and assessment of clinical need, diagnosis and evidenced based psychotherapy for those Veterans in the MHRRTP. The Senior Licensed Professional Mental Health Counselor operates independently. The Licensed Professional Mental Health Counselor will provide specialized treatment to veterans in the MHRRTP and is expected to be trained in and use evidenced based psychotherapies for the treatment of a Substance Use Disorders (SUD) and co-occurring conditions such as Cognitive Behavioral Therapy for Substance Use Disorders (CBT-SUDS), Motivational Interviewing (MI), Motivational Enhancement Therapy (MET), and Contingency Management (CM), and COPE (Concurrent Treatment of PTSD and SUDS Using Prolonged Exposure). Duties include but not limited to: Collaborates with the other members of the interdisciplinary treatment team in the provision of comprehensive mental health services to Veterans, ensures equity of access, service, and benefits to this population and ensures the care provided is of the highest quality. In conjunction with the healthcare team, identifies and assesses risk, safety, supports and implements appropriate interventions/treatment for those who are experiencing abuse, neglect, exploitation or are experiencing domestic or intimate partner violence. Adheres to VA and mandated reporter requirements. Advises and collaborates with interdisciplinary teams throughout the medical center on abuse, neglect, exploitation and intimate partner violence issues. Possesses ability to provide clinical leadership, direction, orientation, coaching, in-service training, staff development, and continuing education related to area of specialization. Expected to serve on committees, work groups, and task forces at the facility, VISN and national level, or in the community as deemed appropriate. Possesses advanced knowledge of and mastery of theories and modalities used in the specialized treatment of SUD and co-occurring conditions. Develops, implements, and documents the psychotherapeutic treatment plan for an assigned caseload of patients, including prompt completion of clinical reminders. Serves as the Mental Health Treatment Coordinator (MHTC) for assigned veterans and conducts the duties of this role when indicated. - Supervises trainees conducting direct clinical care. Provides administrative duties including Managing Return to Clinic Orders and consults for Veterans as needed, returning Veterans' phone calls and secure messages, writing reports, scoring Measure Based Care tools, attending organizational meetings. Attends and participates in administrative and informational meetings held by the ACOS, MH, Deputy ACOS, MH and other managers. Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Mental Health, Joint Commission, CARF and facility standards. Participates in readiness and response to outside accrediting agencies and OIG. Provides consultation and education to Veterans/support systems regarding community resources, VA benefits and specialty programs, and advance directives. This includes the incumbent maintaining knowledge of the process for accessing and/or coordinating community-based services, including information and referral for additional services from other VA programs, other government programs, and community programs. Participates in weekend rotation with other clinical providers. Qualifications Basic Requirements: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. LPMHCs must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3.j. Education. Hold a master's or doctoral degree in: Clinical Mental Health Counseling; Clinical Rehabilitation Counseling; Clinical Mental Health Counseling and Clinical Rehabilitation Counseling; or a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP). Examples of related mental health counseling fields include, but are not limited to Addiction Counseling; Community Counseling; Gerontology Counseling; Marital, Couple, and Family Counseling. NOTE: Traditional Rehabilitation counseling programs that are accredited by CACREP do not meet the LPMHC qualification standards as Traditional Rehabilitation counseling differs from Clinical Rehabilitation counseling. Licensure. Persons hired or reassigned to LPMHC positions in the GS-0183 series in VHA must hold a full, current, and unrestricted license to independently practice as a Licensed Professional Mental Health Counselor, which includes diagnosis and treatment. NOTE: It is VHA policy that a LPMHC who does not yet have a license that allows independent practice must be supervised by a licensed independent practitioner of the same discipline who is a VA staff member and who has access to the electronic health record. Grade Determinations. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Senior Licensed Professional Mental Health Counselor, GS-12 Experience, Education, and Licensure. In addition to the basic requirements, candidates must have at least one year of progressively complex LPMHC experience equivalent to the GS-11 grade level. Senior LPMHCs must have five years of post-licensed experience that demonstrates possession of advanced practice skills and judgment, demonstrating progressive professional competency and expertise and be licensed to provide clinical supervision to trainees or unlicensed LPMHCs. Demonstrated Knowledge, Skills, and Abilities. In addition to meeting the KSAs for the GS-11 grade level, the candidate must demonstrate all of the KSAs below: 1. Advanced knowledge of and mastery of theories and modalities used in the specialized treatment of complex mental illness. Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Ability to determine priority of services needed and provide specialized treatment. 2. Advanced and expert skill in a range of specialized interventions and treatment modalities for Veterans with complex needs. This includes individual, group, and/or family counseling, or psychotherapy and advanced level psychosocial, and/or case management interventions used in the treatment of Veterans. 3. Ability to coordinate the delivery of specialized psychosocial services and programs. Ability to design system changes based on data. 4. Ability to provide subject matter consultation to colleagues and trainees on the psychosocial treatment of patients, rendering professional opinions based on experience, expertise and role modeling effective clinical skills. 5. Ability to teach, mentor staff and trainees, and provide supervision for licensure or for specialty certifications. 6. Ability to engage in written and oral communication with leadership/staff and community stakeholders regarding policies, procedures, practice guidelines, and issues pertaining to the practice of the profession.
