How you make a difference

The **Admissions and Discharge Coordinator** plays a vital role in the seamless transition of individuals into and out of the facility. From receiving and organizing referrals to managing all related communications, this position ensures a smooth admission process while maintaining ongoing contact with community referral sources. By overseeing the coordination of admission and discharge details, the coordinator ensures individuals are properly supported at each stage. Key Responsibilities

+ **Coordination and Communication** : Coordinates the admission process, communicates with receiving facilities, and assists physicians in obtaining necessary information.

+ **Referral Management** : Thoroughly reviews referrals and sets up admissions while maintaining communication with referring agencies and tracking referrals.

+ **Organizing Admission Paperwork** : Organizes all admission paperwork and provides comprehensive reporting on admission activity to Administration.

+ **Risk Management Awareness** : Demonstrates understanding of risk management, infection control, fire and disaster procedures, and abuse reporting.

+ **Occupancy and Bed Management** : Keeps track of facility occupancy, vacancies, discharges, and reconciles bed counts.

Qualifications & Requirements

Education:

+ Required: High school diploma or equivalent.

+ Preferred: Bachelor’s degree.

Experience:

+ Preferred: Two (2) years of admissions and discharge experience in psychiatric setting.
