    Description

    Hours: Part time/20 hours; evening Shift - 4pm- 9:30pm Monday - Thursday or Tuesday - Friday.

     

    This position comes with an additional $10 shift differential on top of our normal differentials.

     

    + Independently assess, coordinates and implements a timely, safe patient discharge plan to the appropriate level of care.

    + Participates in interdisciplinary approach to patient care management.

    + Works with complex patient populations who have high acuity chronic illnesses.

    + Develops treatment plans, provides patient and family education and serves as a professional resource.

    + Provides emotional support and psychosocial counseling to patients.

    MINIMUM QUALIFICATIONS:

    + Masters in Social Work from an accredited Institution.

    + At least 2 years of relevant social work experience preferred.

    + Experience in an Acute Care setting or post-acute care setting desired but not required.

    + Positions in this classification require LMSW license in the state of Georgia.

    + Must have working knowledge of software applications.

    + ACM, CCM preferred.
