Job Overview:

Community Health Worker is an individual who is competent to perform assigned functions of non-medical care to the client in their residence.

Qualifications:

Required:

+ High School diploma or GED preferred

Scheduling Requirements:

+ Days to work

+ FT

Essential Functions:

+ Meet patients recently discharged in their home and conduct needs assessment using Social Determinants of Health Tool.

+ Schedule weekly follow-up calls and home visits to patients.

+ Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives.

+ Assist patients with organizing their records, making follow-up appointments with a Primary Care Physicians or Specialists.

+ Help patients with the tasks of getting medical care, and work on health goals by helping to plan or prepare healthy meals or finding time to exercise.

+ Guide patients with grooming , dressing, taking medications, and housekeeping,

+ Help patient s access resources, including quality of care, childcare services, food, housing, transportation.

+ Maintain relationship with the Community Based Organization.

+ Able to travel and meet the members where they are; may include local Churches, homeless shelter, and members residence.

+ Must have an empathetic attitude toward the care of the sick and elderly to deliver services in accordance with our mission and vison.
