Job Overview:
The Population Health Clinical Manager will lead the development, implementation, and management of population health initiatives,
This role will oversee clinical operations, ensure the delivery of high-quality care, and utilize data analytics to drive improvements in health outcomes for targeted populations,
The ideal candidate will have a strong clinical background and experience in population health management,
Qualifications:
Required:
+ Bachelor's degree in nursing, Public Health, or a related field
+ Minimum of 3 years of experience in clinical operations, population health, or healthcare management
+ Strong knowledge of healthcare systems, patient care models, and population health metrics
+ Proven leadership skills with the ability to motivate and guide a diverse team
+ Excellent analytical, problem-solving, and decision-making skills
+ Effective communication and interpersonal skills, with the ability to work collaboratively in a team environment
+ Proficient in data analysis tools and electronic health record systems,
Preferred:
+ Master's degree preferred
+ RN and certification in population health management or related field preferred
Essential Functions:
+ Develop and implement population health strategies that align with organizational goals and enhance patient care,
+ Oversee clinical programs focused on disease management, preventive care, and health promotion for diverse populations,
+ Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and community partners, to create integrated care plans
+ Utilize data analytics to identify trends in health outcomes, patient engagement, and resource utilization, and implement strategies for improvement
+ Monitor and report on key performance indicators related to population health initiatives,
+ Ensure compliance with regulatory and quality standards in all population health activities,
+ Conduct training and education sessions for clinical staff on best practices in population health management,
+ Facilitate community outreach efforts to engage and educate patients on available resources and health initiatives,
+ Act as a liaison between clinical teams and leadership to advocate for patient-centered care practices and policies,
