**Job Title:** RN Hospital at Home

**Location:** Temple, Texas

**Status:** Full-Time

**Shift:** Days, 9A-9P, 36 hours per week, (3) 12-hours shifts

Our program offers inpatient level of care services in the comfort of the patient's home. We prioritize quality, acute care delivery, ensuring patients can remain at home while achieving positive clinical outcomes.

We are seeking compassionate and skilled Registered Nurses to join our dynamic Hospital at Home team in **Temple, Texas** . As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care to patients within their homes. You will play a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support.

KEY RESPONSIBILITIES

• Provide high-quality acute care services to patients enrolled in the Hospital at Home program.

• Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs.

• Spend additional time at bedside assessing clinical and environmental needs and supporting an overall healing environment.

• Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home.

• Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes.

Qualifications - External

QUALIFICATIONS

EDUCATION - Grad of an Accredited Program

MAJOR - Nursing

CERTIFICATION/LICENSE/REGISTRATION - Registered Nurse (RN)

Basic Life Support (BLS): BLS or BLS within 30 days of hire or transfer.
