About Fort Logan:
The Colorado Mental Health Hospital in Fort Logan (CMHHIFL) is one of two state hospitals serving our community. The vision of CMHHIFL is to provide the highest quality mental health services to persons with complex, serious, and persistent mental illness within the resources available. Our mission is to support those individuals in achieving their recovery goals and reaching their full potential. CMHHIFL provides inpatient treatment to adult patients, generally between the ages of 18-59. The hospital currently has 138 adult inpatient beds and over six treatment milieus serving civil and forensic individuals. Medical services, occupational therapy, neuropsychological-rehabilitation services, and trauma-informed care services are available for all patients throughout the hospital.

About this Work Unit:
The Social Work IV position works in the 24-hour State psychiatric hospital with individuals voluntarily and involuntarily committed for treatment. Patients receiving treatment at CMHHFL have severe and persistent mental illness and may have traumatic brain injury, trauma disorders, and other complex dynamics. Each multidisciplinary treatment team includes psychiatrists, psychologists, social workers, addiction specialists, occupational therapists, recreation therapists, creative arts therapists, nurses, physicians, and mental health clinicians. About the Position:
The Social Work unit collaborates with patients, their families and/or support systems, and external stakeholders to plan, organize, and execute psychiatric treatment interventions and comprehensive aftercare plans with placement. Services include clinical assessment, treatment planning, individual therapy, group therapy, family therapy, counseling, and education. This work unit serves as a liaison between the hospital and Community Mental Health Centers (CMHCs) and other governmental entities.
The purpose of the Social Worker is to provide individual, group, and family therapy. Individual and family therapy are provided as needed, which is determined in the treatment plan meetings and collaboration with the treatment team. Group therapy is offered to all patients, and group assignments shall be determined by clinical and individualized need. The Social Work positions are responsible for developing group materials appropriate for patient-specific needs and leading group therapy sessions. The Social Worker completes comprehensive biopsychosocial assessments and reassessments, coordinates treatment planning for all patients, and regularly documents in the electronic health record. The Social work team is responsible for coordination of discharge and comprehensive aftercare plans. In doing so, this position collaborates with support systems, stakeholders, CMHCs, and other agencies to coordinate continuity of care. The Social Worker actively contributes to hospital-wide events, in-service training, and program development.
The Social Worker IV supervises the social work department staff members and supports execution of the hospital’s mission, vision, and values within the social work department and in collaboration with leadership. As a supervisor, the Social Worker IV is responsible for clinical and administrative supervision, as well as performance management and disciplinary processes. The Social Worker IV provides clinical leadership by contributing to, reviewing, and overseeing social work teams' treatment plans, documentation, assessments, and therapeutic interventions. This position is also responsible for overseeing various departmental and discipline-specific projects and supports implementation within the team. Duties in this position include:
Assessment, Intervention, and Treatment Planning:
+ Responsible for individual therapy, group therapy, and family therapy. Individual and family therapy are provided as needed, which is determined in the treatment plan meetings and collaboration with the treatment team. Group therapy is offered to all patients and group assignment shall be determined by clinical and individualized need.
+ Responsible for developing group materials appropriate for patient-specific needs and leading group therapy sessions. When providing such treatment, the position relies upon social work theories and concepts relevant to the needs of the patient and their support systems to support symptom reduction. Interventions are individualized and tailored to issues related to age, ethnicity, culture, religion, gender identity, and sexuality.
+ The social work team communicates treatment needs, barriers, expectations, and changes in behavior to the individual's support system throughout their admission cycle.
+ Provides psychoeducation to an individual's support system and community stakeholders in an effort to solicit understanding and participation in the treatment process.
+ Monitors the effectiveness of various treatment interventions tailored to reach specific treatment goals and objectives by leading the treatment planning meetings for all patients and ensuring goals are specific, measurable, attainable, relevant, and time-based (SMART) and in accordance with regulatory and licensing requirements, as well as best practices.
Case Management and Discharge Planning:
+ Responsible for overseeing all team members' progress and coordinating resource needs. This may include monitoring submission of benefits, identifying appropriate benefits, guardianship needs, Momentum referrals, obtaining identification, securing a payee, applying for Medicaid/Medicare, and submitting referrals to potential discharge locations.
+ Works closely with the benefits specialist and reintegration specialist, assists in the completion and monitoring of such documents and referrals, and may collaborate with legally appointed guardians or members of the patient’s support system to obtain the necessary information and navigate the process.
+ Leads the team in identifying appropriate discharge locations in collaboration with the treatment team and liaisons with those placements to support continuity of care.
+ Coordinates day passes off-campus, attends placement visits, coordinates family and other visits, and attends other off-campus appointments as needed.
+ Attends interagency staff meetings often and is responsible for providing education to stakeholders regarding the Social Work Department and building relationships across various agencies.
+ Supports team members in navigating various systemic barriers and ensuring timely and accurate processes are completed. All efforts are geared toward supporting discharge into the least restrictive environment.
Specialty Program and Project Implementation:
+ Responsible for creating, implementing, and overseeing designated specialty programs and/or departmental projects. This may include specialized treatment modalities, including but not limited to Animal Assisted Therapy, Substance Addiction Treatment, Virtual Reality programs, and Peer Programs.
+ Partnering with community stakeholders for shared resources and development of discipline-specific training opportunities. At times, these duties require liaising with community stakeholders, community site visits, creating and sharing information materials or sessions, establishing protocol and policy, training staff, and overseeing and auditing the implementation and outcome measures.
Social Work and Substance Abuse Education Department Supervision:
+ Responsible for employee supervisory responsibilities within the work unit and hospital-wide CAS services. This includes provision of clinical and administrative supervision, professional development, employee performance evaluations, employee performance plans, corrective and disciplinary actions, assist resolving informal grievances, staff scheduling, position descriptions, recruitment, retention, interviewing and hiring, onboarding, and training, and ensuring hospital wide required training has been completed.
+ Facilitates newly hired social workers and CAS orientation, in collaboration with the social work department and other disciplines.
+ Responsible for completion of department-specific audits to support high-quality documentation, assessment, report writing, and treatment.
Student Training and Student Supervision:
+ CMHHFL serves as a training site for social work externship programs. Responsible for building relationships with academic institutions, creating training opportunities appropriate to skill level, and creating didactic curriculum.
+ Supervises members of their team in the supervision of the externship students (supervision of supervision).
Other Duties as Assigned:
+ Other duties as assigned include hospital-wide ad hoc activities, milieu support, etc.
Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Education/Licensure/Certification/Experience:
Current, valid Colorado or a State participating in the Social Work Licensure Compact licensure as:
+ Licensed Social Worker (LSW) OR
+ Licensed Clinical Social Worker (LCSW) OR
+ Licensed Professional Counselor (LPC) OR
+ Licensed Marriage and Family Therapist (LMFT)
+ Licensed Addiction Counselor (LAC)
Two (2) years of experience with current, valid licensure from a state participating in the Social Work Licensure Compact based on the work assigned to the position.
Preferred Qualifications:
+ 3 years post LCSW experience focused on treating individuals with severe and persistent mental health issues, trauma, and/or substance abuse
+ 1 year providing clinical supervision
+ 1 year of working with patients in a secured setting
+ Licensed Clinical Social Worker
+ LAC preferred
+ Knowledge of DSM 5-TR
+ Prior adult inpatient experience
+ Desire to work with adults who have been diagnosed with severe and persistent mental illness
+ Experience working with clients who have dual diagnosis issues, substance abuse, and mental health issues.
+ Experience and/or desire to work with diverse, under-served client populations.
+ Previous State Employee
