    Peckham Inc. - Activity Care Facilitator/Direct Care Staff

     

    SUMMARYThe Activity Care Facilitator / Direct Care Staff will provide compassionate, person-centered support to six adult men with developmental disabilities in a licensed Adult Foster Care setting. This position is responsible for delivering high-quality direct care services while also planning and leading engaging, meaningful activities in both home and community environments. By bridging daily personal care with enrichment opportunities, the facilitator promotes skill-building, social connection, and an enhanced quality of life for each resident. The role also ensures compliance with licensing standards and fosters a safe, supportive, and family-like atmosphere. Candidates must possess the physical, emotional, intellectual, and social stamina necessary to meet the diverse needs of the individuals in their care.

    MAIN DUTIES AND RESPONSIBILITIES

    + Design, organize, and lead daily and weekly activities that reflect the interests, abilities, and goals of both individual residents and the group as a whole.

    + Develop and maintain a diverse and inclusive activity calendar for the House of Ruth, ensuring consistent, meaningful engagement across a variety of interests and skill levels.

    + Plan ,coordinate, and facilitate community-based outings that promote social interaction, personal development, and fulfillment of goals outlined in each resident’s Individual Plan of Service (IPOS).

    + Comply with Adult Foster Care licensing rules, CMH policies and guidelines, House Guidelines, Peckham/House of Ruth handbooks, HUD guidelines (including performance of safety drills such as fire, tornado, electrical, flood, etc).

    + Demonstrate sensitivity and compassion when engaging with residents, while maintaining appropriate confidentiality and ensuring accurate documentation of resident records, including person-centered plans, daily logs, medication records, safety drills, incident reports, and other required documentation.

    + Promote resident growth by encouraging cooperation, self-esteem, independence, normalization, and the development of positive social skills through all interactions and activities.

    + Supervise, protect, and assist residents with personal care needs as outlined in their assessment plans, including intensive support such as lifting and transferring when necessary.

    + As specified in the resident assessment plan, provide direction and opportunity for growth and development of independence, including involvement in educational, employment and day programs

    + Foster resident relationships and a sense of family to include relatives and friends

    + Meal preparation in compliance with dietary considerations

    + Dispense medications as prescribed

    + Monitor sleep hours for resident safety and accountability

    + Housekeeping, general home maintenance, and laundry

    + Oversee any required rehabilitation exercises

    + Communicate with parents/guardians as appropriate

    + Prepare HUD, Adult Foster Care License and Community Mental Health paperwork and reports

    OTHER DUTIES AND RESPONSIBILITIES

    + Maintain a safe, clean, and welcoming living environment.

    + Promote Peckham’s vision, values, and mission to all stakeholders.

    + Assist with maintaining organizational-wide quality standards.

    + Perform other related duties as assigned.

    SUPERVISORY RESPONSIBILITIES

    + None.

    MINIMUM QUALIFICATIONS

    + High School Diploma or GED

    + Must maintain an unrestricted Michigan driver’s license and possess a satisfactory driving record per the company auto insurance

    + Reliable transportation

    + Ability to obtain First Aid, CPR, company and group home curriculum trainings

    + Ability to complete Basic Health and Medication course

    MINIMUM QUALIFICATIONS

    + 1month of related experience.

    Travel Requirements

    + This role will require offsite travel at least 40% of the time.

    PHYSICAL DEMANDS

    This position involves frequent physical activity, including standing, walking, bending, and engaging in physical play. Tasks may also include occasional lifting at least 50, carrying, running, and moving furniture, as well as light cleaning activities like sweeping, mopping, and vacuuming. Employees will frequently communicate with and actively supervise others, requiring strong coordination. The work environment is dynamic, with frequent exposure to noise, active settings, and occasional use of cleaning agents. Conditions may involve rare exposure to temperature changes, confined spaces, or allergens.
