    Essential Job Functions

     

    As an Occupational Therapist, you will support clients in maximizing their independence and improving their quality of life through personalized therapy interventions in the home setting.

     

    • Conduct patient evaluations and develop customized care plans focused on functional goals.

    • Deliver skilled therapy to help clients improve ADLs, IADLs, strength, coordination, and safety.

    • Educate patients and caregivers on safe practices, equipment use, and home modifications.

    • Collaborate with physicians, nurses, and other care team members.

    • Maintain accurate documentation and assist with certification/recertification paperwork.

    • Support quality improvement efforts and participate in optional on-call rotations.

    Requirements

    • Active Occupational Therapist (OTR/L) License in Minnesota

    • Two-Step TB Test (provided if needed)

    • DHS Criminal Background Clearance (we provide screening)

    • Valid Driver’s License & Auto Insurance
