    Description

    THE POSITION: The pharmacy clinical coordinator is responsible for maximizing the optimal delivery of pharmacy services related to the care of patients at Ventura County Medical Center and Santa Paula Hospital. Reporting to the Director of Pharmacy, a full-time clinical coordinator will be responsible for developing, implementing, and sustaining clinical pharmacy programs across the hospital with interdisciplinary programs and teams. They will champion variations of reduction efforts, provide oversight and guidance on medication use, quality assurance and compliance with emphasis on patient safety.

     

    The clinical coordinator is responsible for development, implementation, and maintenance of clinical pharmacy services, advance and manage the Antimicrobial Stewardship Program, provides staff/pharmacy student education and training, coordinate medication use evaluation (MUE), and is an active member of a multidisciplinary group through committee involvement for creation of clinical practice guidelines and hospital policies. Promote safe medication usage through research, strategic planning, staff development, and exemplary leadership.

     

    PAYROLL TITLE: Pharmacist - Specialty Care

     

    EDUCATIONAL/BILINGUAL INCENTIVE: Incumbents may be eligible for an educational incentive of 2.5%, 3.5%, or 5% based on completion of an Associate's, Bachelor's, or Master's degree that is not required for the classification. Incumbents may also be eligible for bilingual incentive depending upon operational need and certification of skill.

     

    The eligible list established from this recruitment may be used to fill current and future Regular, (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for this and similar vacancies within the Ventura County Health Care Agency. There is currently one (1) Regular, Full-Time vacancy within the Health Care Agency - Ventura County Medical Center and/or Santa Paula Hospital.

     

    Pharmacist - Specialty Care is represented by the Service Employee International Union (SEIU) and is eligible for overtime compensation.

    Examples Of Duties

    Duties may include but are not limited to the following:

    + Develops and revises policies and procedures or clinical practice guidelines to optimize drug utilization and medication safety while meeting accreditation standards and regulations. The accreditation standard includes but is not limited to The Joint Commission (TJC), California Department of Public Health (CDPH), California Board of Pharmacy (CABOP), Center for Medicare and Medicaid Services (CMS), and National Healthcare Safety Network (NHSN).

    + Oversees the pharmacy practice program by ensuring initial and on-going competency-based assessment of pharmacy staff, aligning their skills with the delivery of exceptional patient care and supporting the hospital’s mission and goal.

    + Provides leadership for various clinical education programs for pharmacy students, pharmacists and other healthcare professionals.

    + Displays positive leadership traits. Serves as a role model, through creativity, cooperation, innovation, and peer support. Works to maintain high morale within the department.

    + Ensures that pharmacists and pharmacy technicians are properly trained.

    + Proactively works to identify and meet staff development needs for the department.

    + Meets with staff to voice concerns or ideas.

    + Demonstrates the clinical skills and professional judgement necessary to lead and direct pharmaceutical care services.

    + Manages the Antimicrobial Stewardship Program as an active member of the ASP Committee by spearheading committee meetings, gathering and reporting metrics to NHSN, complete annual antibiogram, and maintain facility’s Honor Roll designation awarded by CDPH.

    + Develops and presents therapeutic class review for medical staff review to include utilization, financial analysis, contract compliance, safety and efficiency.

    + Provides oversight on formulary review process for Pharmacy and Therapeutic’s Committee (P&T).

    + Coordinates MUE with Specialty Care Pharmacists.

    + Reviews system medication variances and falls related to medication. Analyze trends, identify process improvement opportunities, and implement change.

    + Develops and revises medication related policies and procedures to ensure the department meets requirements of Board of Pharmacy, The Joint Commission, and legal regulatory requirements.

    + Coordinates and manages pharmacist intervention program.

    + Understands the principles and concepts of Performance Improvement. Seeks opportunities and solutions for process improvements and to improve job skills of staff.

    + Represents the pharmacy department on medical staff committees to assure rational drug therapy decisions are made in the best interest of the healthcare system.

    + Serves as student program coordinator for colleges of pharmacy for pharmacy practice rotations.

    + Performs related duties as required (including staffing).

    Typical Qualifications

    These are entrance requirements to the examination process and assure neither continuance in the process nor placement on an eligible list.

    EDUCATION, TRAINING, and EXPERIENCE:

    + Pharmaceutical degree (Bachelor of Science in Pharmacy) from an accredited school of pharmacy or its equivalent plus a minimum of a combination of three (3) years of experience as a hospital inpatient staff pharmacist, OR

    + One (1) year of specialized clinical pharmacy experience, OR

    + Successful completion of a pharmacy residency program in clinical pharmacy.

    NECESSARY SPECIAL REQUIREMENTS:

    + Must possess and maintain a current valid license as a Pharmacist in the State of California

    + Willing and able to lift up to 25 lbs.

    KNOWLEDGE, SKILLS AND ABILITIES:

    Thorough knowledge of: prescription filling activities; unit dose systems; parenteral nutrition dosing and management; sterile and non-sterile compounding; pharmacology and pathophysiology; professional standards and practices; Federal, State Board of Pharmacy, The Joint Commission and Department of Public Health laws, regulations, and guidelines pertaining to patient care; sterile compounding of non-hazardous and hazardous medication.

     

    Skilled in: Data analysis including proficient use of Microsoft Office programs; written and verbal communication.

     

    Working ability to: operate a computer; perform pharmaceutical mathematical calculations; research, read, interpret medical and pharmacy literature; communicate clearly and effectively; review and verify medication orders accurately and timely through electronic health record; use aseptic technique for non-sterile and sterile compounding of non-hazardous and hazardous medications; supervise pharmacy technicians; work cooperatively with other staff and health care providers; participate in didactic and/or clerkship teaching; and conduct scholarly writing and research; and willing and able to lift up to 25 lbs.
