    Responsibilities

    A CTS navigator is an asset to the program and provides improved patient experience, cost reduction, and enhanced operational efficiency. The navigator role has demonstrated improved patient outcomes, increased patient satisfaction, and strengthened the program and facility reputation and revenue streams.

     

    Patient Care: Meet patients/families who have been or potentials to be referred for cardiothoracic surgery. Serves as the patient liaison throughout the care continuum. Helps coordinate patient care. Serves as an ongoing resource for patients and families during the decision, treatment and rehab period. Communicates information regarding patient status and needs. Collaborates with other care providers regarding how best to meet needs and sequence care for improved efficiency and LOS. Coordinates admission process for patients when appropriate. Collaborates with community resources to provide assistance to patients/families for patient needs. Work closely with case manager.

     

    Continuity of Care: Reviews and prevents medically unnecessary patient admissions through chart review of patient assessments on follow-up care. Responsible for post-op clinic and early identification of complications to mitigate ED returns or readmissions.

     

    Development of Standards: Develops and monitors clinical quality indicators. Develops patient tracking database and assists with analysis and application in partnership with PI coordinator. Develops action plan for indicators that fall below established benchmarks in partnership with PI coordinator. Participates in the development of and compliance with treatment guidelines. Partners with the PI coordinator in the assessment of hospital practice patterns, identifying needed changes, establishing action plans and identifying outcomes.

     

    Leadership: Communicates effectively, timely and in an open, honest manner. Shares knowledge and skills with colleagues and others. Recognizes, respects and shows trust in colleagues and their contributions.

     

    Out-Patient Functions (if applicable): Coordinates and attends multi-disciplinary meetings Works with surgery clinic to ensure transitions of care needs met and efficiency of post-op clinic.

    Qualifications

    + Education: 4 Year / Bachelors Degree, preferred.

    + Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.

    + Licensure/Certification: Current nursing license in appropriate state, required.

    + Additional skills required: Ability to successfully complete generic and department-specific skills validation and competency testing.
